❓ How can home health and hospice agencies be sure their patients are in the right care setting for them and their families? 🔎 Take a look at our latest article to find out! https://lnkd.in/gqupD2Jr Coordination of patient transitions and understanding which patients are best suited to hospice care can be challenging. Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning. Predictive analytics can help agencies do just that: https://lnkd.in/gqupD2Jr #homehealth #hospice #predictiveanalaytics #comfortcare

Let’s get some misconceptions about utilization management in hospice out of the way. What comes to mind is often that it’s ‘stingy’ or lacking in compassion. In reality, it’s quite the opposite. Providing compassionate hospice care that puts the needs of our patients and their families first *necessitates* utilization management. By carefully managing our resources, we can offer personalized care plans that cater to each individual's unique needs and preferences. Our team works tirelessly to ensure that every patient receives the highest quality of care, making their comfort and well-being our top priority. Together, we strive to create a supportive and nurturing environment where everyone can receive the care and attention they deserve during their end-of-life journey. #HospiceCare #UtilizationManagement #CompassionateCare #BestCare #EndOfLifeJourney

In today's healthcare system, reflecting on lessons learned and educating the population I serve are key priorities. This weekend, I encountered a patient whose goals were neglected, physicians weren't listening, and his sister was advocating tirelessly for him. In Value-Based Care, discussing patients' goals at every visit is paramount. In hospice, we adapt care plans daily to align with patient and family goals. Recognizing the importance of timely hospice care within the broader value-based care model is essential. As healthcare professionals, we must prioritize integrating hospice care into value-based models. This ensures that patients receive compassionate care tailored to their needs and preferences. Let's advocate for care models that prioritize quality, efficiency, and, most importantly, the dignity and respect every patient deserves. #Healthcare #ValueBasedCare #HospiceCare #PatientCenteredCare #HealthcareLeadership #QualityCare
