On World Hypoparathyroidism Awareness Day, we want to increase awareness and understanding of hypoparathyroidism, its treatment, and its impact on patients and their families. Hypoparathyroidism is a rare endocrine condition when the parathyroid glands in your neck are inactive or produce too little parathyroid hormone (PTH) – which leads to low blood calcium levels (hypocalcemia). Calcium is in every cell and is vital for the functioning of: 📌nerves 📌muscles 📌bones 📌kidneys 📌eyes 📌heart In hypocalcemia, calcium levels need careful management and regular monitoring. Hypoparathyroidism may be linked to multiorgan effects, e.g., cardiovascular, renal manifestations. 📌When hypoparathyroidism occurs as a complication of surgery, symptoms may start soon after surgery. 📌In the case of a non-surgical type of hyperparathyroidism, diagnosis can take a lot longer as these conditions are even rarer, so hypoparathyroidism can be hard to identify. New research unveiled at the European Society of Cardiology Conference 2024 links Ozempic and other GLP-1 drugs to slowing down biological aging. The effect these drugs have on diminishing an individual’s inflammatory state is especially exciting because while inflammation is a part of wound healing, an over-exuberant inflammatory response or baseline state of inflammation can actually impair and slow wound healing and lead to more significant scarring. If we can have our patients in a less inflammatory state at the time of surgery, it can do only good things for the healing process and the outcomes of the procedures that they’re going to be having. A required two-week pause prior to anesthesia for a surgical procedure should not undo these effects. 🥊 TAP vs QL? Who wins this battle? 🥊 Postoperative pain management is critical for improving recovery after laparoscopic abdominal surgery. This randomized clinical trial, just published in RAPM, compares two widely used techniques: lateral quadratus lumborum (QL) blocks and transversus abdominis plane (TAP) blocks. Here’s what the authors found: 👉 QL blocks achieved greater dermatomal anesthetic spread, with patients more likely to experience sensory loss at higher levels (above T8 and T10). 👉 TAP blocks were more likely to result in indeterminate anesthesia, highlighting the variability in their effectiveness. 👉 Surprisingly, despite these differences, pain scores and opioid consumption at 24 hours were similar between the groups. These findings spark important questions: • Is dermatomal spread the key to better postoperative analgesia? • How can we optimize these blocks to further reduce opioid use and enhance recovery? American Society of Interventional Pain Physicians (ASIPP) just published one of the largest regenerative Medicine textbooks (Second Edition). Laxmaiah Manchikanti, MD is the main editor and I was honored to be one of the co-editors. In this Knee chapter, we discuss the role of PRP and Bone marrow concentrate to treat various knee injuries and osteoarthritis. The focus is on the different techniques to inject the joint, meniscus, MCL, LCL, etc. Emphasis was given to the advanced fluoroscopic guided subchondral/intraosseous Knee procedures (femoral and tibial) that are required to treat moderate to severe osteoarthritis. 🌟 Advancing Pain medicine with Regenerative Medicine 🌟 Today, I had the honor of treating a 65-year-old lady suffering from hip osteoarthritis, a condition that significantly impacted her mobility and quality of life. Using fluoroscopy guidance, I performed an intra-articular hip injection of platelet-rich plasma (PRP) following a precise joint arthrogram and after radiofrequency hip denervation without administration of steroids after RFA This minimally invasive procedure combines regenerative medicine with targeted pain management, offering hope for improved joint function and reduced pain in patients who may not yet be candidates for surgery. Patients reporting moderate postoperative pain on postoperative day 2 after cardiac surgery are at highest risk of CPSP, and should receive more intensive in-hospital pain management as well as postdischarge follow-up with a transitional pain service.
