In her new Forefront article, Laura Keohane from Vanderbilt University discusses how, as policy makers consider how to expand integrated Medicare and Medicaid coverage among dual-eligible beneficiaries, it is worth considering what lessons might be learned from strategies for expanding Medicaid home- and community-based services (HCBS). "Most HCBS benefits such as personal care services, adult day center services, and assisted living benefits are offered at state’s choice. Even though the Olmstead decision may motivate states to expand HCBS to avoid legal liability, there is no federal legislative mandate for states to offer comprehensive HCBS. The steps that the federal government took to promote HCBS expansion—accommodating gradual change, optional use of managed care, and funding formulas tied to clear, reasonable goals—may be adapted to increasing access to better Medicare and Medicaid coordination. Integration efforts could also learn from the persistent challenges that the HCBS sector has faced in finding enough workers to meet the needs of Medicaid participants." Medicaid agencies across the map are prioritizing the need to effectively engage members to inform program and policy design. Given proposed CMS rules for establishing new types of member advisory groups, many states are seeking to strengthen Medicaid member engagement. In response, CHCS is launching a 14-month initiative to help states develop a member advisory group approach that centers the lived experience of Medicaid members in shaping program and policy decisions. Participating Medicaid agencies will learn from their peers and national experts on promising practices to ultimately implement a member-informed advisory group.
