Today marks the 11th anniversary of the Mastercall Healthcare ‘Pathfinder/ATT’ 24/7 service supporting NWAS Paramedic crews in Stockport and Trafford with Alternatives to conveyances to A&E. ‘Call before you Convey’ Services, as it has recently been redubbed by NHS England's ‘Care Coordination’ guidance, highlight the importance of conveyance and dispatch avoidance schemes to improve patient outcomes, release ambulance resources and improve hospital flow. With an average 88-90% alternative outcome rating, in 2017 Mastercall/NWAS also took the service one step further to train care homes in when to avoid using 999 direct support to a clinician advice line (under the NWAS Pathfinder algorithm). Again, in 2020 this scheme developed further to take 999 CAS direct visiting requests to further aid community care and admissions avoidance. Our ‘alternative outcome ratings’ for this triple pathway service remain at the 90th percentile. With over 1000cases per month, c880cases receive alternative care. Cases are triaged within 20mins, releasing the crew to a pressing emergency; a GP or AP visits within 2hrs as/if needed. Moreover, as a social enterprise provider the service has only ever been part funded. With NHSE focusing on care coordination, we must remind ourselves of the service models that are reliable, tested and efficiently resourced (with capacity to expand) and consider standardising offerings like this in all localities were possible. #CallBeforeYouConvey #UrgentCareInnovation #CareCoordination #HospitalFlow #PatientOutcomes #AdmissionsAvoidance #UrgentCareTransformation #EmergencyResponseInnovation

🚑 Happy #InternationalParamedicsDay! This year's theme, #TheDifferenceWeMake, couldn't be more relevant for the performance and financial recovery of the NHS and UEC. We think that the Ambulance service has a critical and often underutilised role in supporting health system flow, performance, and financial delivery. We've seen performance and financial improvement plans across the country. Most are single-provider documents with a skewed focus on managing the symptoms of problems. This makes sense because managing what’s in your control is easier, but it doesn’t solve the actual issues. Changing flow to improve outcomes, improve performance and reduce cost is achievable. But it requires system thinking, a detailed understanding of the moving parts and a bit of personal risk. ❓ So what's missing? Often it's the early engagement and involvement of the ambulance service in solving the UEC challenges. Too often, we've seen the ambulance service have a seat at the table but no voice in the decisions. And that's a huge missed opportunity. Ambulance services are often the first point of contact for people in need. The paramedics have the first, and frequently the best, chance to avoid a hospital admission by directing, connecting and taking people to alternative services. When that’s not possible, their clinical skills can be used to get patients to the right place the first time, avoiding A&E and reducing the burden on the health system. Sometimes, they'll get it wrong—errors are part of being human—and this may create risk. But risk is relative and dynamic. Does a stable person taken incorrectly to an SDEC unit create more risk than adding another person to a crowded A&E queue to wait to be seen? We've heard countless paramedics say, "We know this [A&E] isn't the right place, but we're not allowed to take them where we think they need to go." Isn't it time we changed that? 🧠 Picture this: It's the middle of the night, someone's in crisis, and there is no family around. The person needs to be admitted, and the A&E staff already think this may be a complex discharge. Who better to feed into discharge planning than the paramedics who've just been in that person's home? Who have already seen the access route, the house's space, the living set-up and the keysafe on the wall. 💡 So here's our challenge to you: If the ambulance service isn't already at your table, helping you to create combined UEC performance and financial recovery plans, get a call in with them next week. Elective recovery ❤️🩹 is a crucial objective for the NHS. Rising demands for emergency care can crowd out the capacity for accessible and effective planned care. And yet planned care is crucial to honour the NHS constitution and to proactively support people’s health needs promptly – ultimately reducing demands on emergency care in the first place. Although there is a clear need to drastically reduce the current backlog and improve elective activity across the NHS, growing demand, workforce issues and funding constraints makes this an incredibly challenging task. And in an election year, these challenges are only going to gain prominence. Last year, Manchester University NHS Foundation Trust (MFT) had the largest patient waiting list and the longest waits for elective care of any provider in the country. We worked with them to maximise the use of their Trafford Elective Hub, utilising patient insights and improving theatre productivity to increase elective activity. Our work with the fabulously dedicated team at MFT highlights the power of management consulting and service leaders working in collaboration to deliver notable improvements. NHS England recently reported that A&E departments are facing what could be their busiest summer on record, with 4.6 million attendances in June and July - more than any previous year. From mobile clinics that bring care closer to home for early-stage symptoms, to high-capacity medical solutions which can be integrated directly within the hospital estate, we're here to help with easing pressures by providing space to support with: ✔️ Increasing access to primary care services through community-based clinics ✔️ Utilising dedicated Urgent Treatment Centres (UTC) to manage non-emergency cases ✔️ Enabling hospitals to prioritise and triage urgent and critical admissions, without increasing patient waiting times in A&E Southend University Hospital deployed a temporary mobile solution to reduce patient handover delays and enable ambulance crews to get back out into the community quicker. Very proud to be supporting the great work being done at East Lancashire Hospitals NHS Trust. The digital front door and virtual waiting room has not only saved vital clinical hours but has also made the waiting room safer for patients. Over the past few months our NHS England Streaming & Redirection team have been working with East Lancashire Hospitals NHS Trust and Strata Health UK to develop a case study evidencing the impact the digital front door has had on the trusts performance and patient/staff experience. The booked appointment model means patients with lower acuity conditions are able to wait in a place of comfort until their appointment. Patients complete a clinical assessment and receive a booked appointment in as little as 5-minutes using the NHS England digital product and Strata Health UK's booking software via a Booking and Referral Standard (BaRS) referral. This model and digital assessment has led to a reduction in the average time spent in the department per patient, less patients in the waiting room and improved 4-hour performance.
