Decentralized clinical trials (DCTs) have the potential to dramatically improve patient access, enrollment, trial speed, data quality, and representation. So why has the rollout of DCTs slowed in the last couple of years? One of the main issues is that sites and sponsors are facing challenges with adoption of decentralized elements, like lack of budget and training resources and hesitance to integrate new technologies. Now, industry leaders are weighing in on the future of DCTs, including insights on both their shortcomings and powerful potential to change the entire landscape of clinical research for the better. Celebrated on 20 October, World EBHC Day is a worldwide initiative to create awareness of the need for better evidence to inform healthcare policy, practice and decision-making in order to improve health outcomes globally. "When we sit in front of a doctor, we often tend to think that they have the solution to all our problems, but that's not the case. Evidence-based medicine (EBM) does not work that way. EBM is defined as the explicit, conscientious, and judicious use of the best current scientific evidence (a neologism from the English term 'evidence,' which refers not to what is 'evident,' but to what 'is found after being searched for'—that is, research findings) to make decisions regarding the treatment of an individual patient or a population. Evidence-based clinical practice consists of three main elements that integrate with each other: • the best research evidence • the clinician's clinical experience • the patient's values and expectations This means that the optimal process for making a clinical decision requires knowledge of the best available scientific evidence, combined with the values and preferences of the patient, who is actively involved in the choice of treatment.
