Medication Alert: Substitution approved for gliclazide 30mg SR tablets The Therapeutics Goods Administration (TGA) have announced that since there are ongoing shortages of gliclazide 30mg SR tablets, they have made a Serious Scarcity Substitution Instrument allowing pharmacists to substitute the 30mg SR tablets with the 60mg SR tablets. The pharmacist can dispense these from the original prescription and the GP does not need to write a new prescription for this. In aged care this will mean that the tablets will be halved prior to packing in the relevant dose administration aid. As they are slow release they cannot be crushed or chewed even if they have been broken in half. It is anticipated normal supply will be available after the end of June 2024. #agedcare #agedcareaustralia #medicationmanagement #medication

The emergence of nalmefene as another opioid reversal agent took us by surprise. What do we know about it? ✌🏻 Two available formulations Opvee is the intranasal formulation, with each dose delivering 2.7mg. Zurnai is the auto injector formulation, with each dose delivering 1.5mg. ⏰ Longer half-life The half-life of nalmefene can be as long as 11 hours, compared to 2 hours for naloxone. 💥 Higher potency Studies suggest that nalmefene is approximately 5x more potent than naloxone. 🤓 About the DDM in Opvee I find it very interesting that the intranasal formulation contains an ingredient called dodecyl maltoside (DDM). The addition of DDM shortens the time to peak from approximately 2 hours to minutes! 🤨 But its role… 📌 ACMT/AACT Joint Statement ( https://lnkd.in/e77SB93q. ) The statement recommends naloxone as the first-line treatment due to several concerns: 1. Prolonged withdrawal from nalmefene 2. Lengthened hospital stays due to prolonged withdrawal and the associated costs 3. Lack of robust data on nalmefene Will there be a subset of the population who may actually benefit?
