Important new brief out of the Bipartisan Policy Center: "The Future of Acute Hospital Care at Home" To ensure beneficiaries can continue to receive various levels of care in their preferred settings, the BPC brief provides immediate-term federal policy reform recommendations for the potential reauthorization of the Acute Hospital Care at Home model, including extending the waiver for five years, while outlining key issues for policymakers to consider as further data emerges. ACHP joins the Advanced Care at Home Coalition in calling on Congress to extend the CMS Acute Hospital at Home waiver past its expiration date at the end of the year. A recent Centers for Medicare & Medicaid Services report shows that the Hospital at Home model improves patient outcomes while ensuring long-term access to care that is delivered in the comfort of patients’ homes. A five-year extension is crucial for continuing to collect data that will shape the future of a permanent care pathway and improve access to underserved communities. With bipartisan support and more than 400 hospitals already participating, extending the waiver offers an opportunity to enhance care delivery at home. Almost half of rural hospitals had negative total margins in 2022 and negative patient care margins both before and after the #COVID19 pandemic, according to a report prepared for the American Hospital Association by faculty at the Virginia Commonwealth University College of Health Professions. When provider relief funds are excluded from margins, the average total margin for rural hospitals was lower in 2022, the most recent year with data available, than in any year since 2017.

Had the privilege of participating in a Structured Judgment Mortality Review (SJR) at my Trust Bolton NHS Foundation Trust. This insightful process involved both qualitative and quantitative reviews of patient care during their last admission. The retrospective nature of the review allowed me to reflect deeply on my own practices and identify areas for improvement. By learning from these cases, we can spot trends in patient care, enhancing our management plans and ultimately improving future outcomes. While we can't change the past, this valuable learning experience empowers us to elevate the standard of care for our patients moving forward. #NHS #Healthcare #PatientCare #ContinuousImprovement #LearningFromExperience

Hot off the press… US Government agency confirms that United Healthcare is manufacturing chronic care diagnoses to increase their profits. Stat news recently performed its own investigation finding that United was sending nurses to homes under the guise of trying to provide earlier interventions for chronic diseases to improve long term health and reduce long term costs. Stats investigation detailed billions in payments from Medicare to United for these questionable diagnoses. The OIG report released today confirms Stat’s reporting and similarly finds that these home visits are creating dubious diagnoses that drive profits without any clear or apparent attempt to treat them, in contrast to the public facing statements by United Healthcare. Let’s reinvest these stolen dollars back into the healthcare system and Part B where they are coming from so we can create a more sustainable future for outpatient physician care. Our recent reports show significant challenges in the hospital discharge processes, with providers caring for over 31,000 people and representing 21,000 care workers. Our results show that 55% say report discharge paperwork doesn't reflect patients' needs, and 63% say early discharges lead to readmissions. But there's some optimism - 69% of providers want to collaborate on solutions. We've presented 35 key recommendations to NHS leaders and government to enhance discharge processes and improve patient outcomes.
