The Centers for Medicare & Medicaid Services (CMS) released the 2025 Proposed Physician Fee Schedule, which includes proposed reimbursement for pre-hospital whole blood. This is incredibly exciting news! Recent research published by Levy, Grafinkel, May, et al., in the Journal of the American College of Emergency Physicians Open (April 2024) highlights that as of March 2024, about 99% of EMS systems in the United States do not carry whole blood products. This proposal is a significant step forward and could lead to faster adoption of lifesaving pre-hospital whole blood treatment across the country. We would like to know if Emergency and Trauma Nurses are ready to receive pre-hospital patients receiving whole blood. Hospitals, States, and Regional Trauma Systems across the country have been striving to implement whole blood initiatives, particularly for emergency and trauma patients who need lifesaving pre-hospital whole blood for hemorrhagic shock. Studies have shown that treating both military and civilian patients in hemorrhagic shock with whole blood transfusions improves patient survival rates compared to those who receive blood component transfusions (Spinella PC et al., 2009; Nessen SC et al., 2013; Hazelton JP et al., 2022). Clinical data supports that rapid administration of whole blood in the prehospital setting can increase survival rates by up to 60% and decrease the need for additional blood products within the first 24 hours by 7% (Torres CM et al., 2023; Brill JB et al., 2022; Guyette FX et al., 2021). This NCIA story describes a case where a patient in an LTACH who had suffered many serious injuries in a motor vehicle accident needed emergent dialysis and Vascular Wellness promptly placed a Vascath; then ten days later, a Tunneled Permcath was ordered after determining the patient would need long-term dialysis, and Vascular Wellness was able to promptly exchange Vascath for a Tunneled Permcath at the patient’s bedside. The irony embedded within the term "Patient" is quite poignant. It's disheartening when the caregiver realizes they must exhibit patience despite being in the role of a nurse. The individual who should be exercising patience in their journey towards recovery or prognosis often struggles with impatience, particularly after receiving the first dose of Emal injection. This raises the question of who truly initiated the designation of "patients" for those under our care in hospitals. It's a sentiment shared by my colleagues, who witness the expectation for immediate change after medical interventions like doses or surgeries. Patients tend to overlook the gradual nature of healing, expecting instant results akin to stat doses. It's a point where the roles seem reversed – perhaps the term "nurses" should apply to the patients, as it's the caregivers who truly embody patience. As for whether patients demonstrate patience towards their caregivers and care, it's a matter that prompts reflection and discussion among healthcare professionals.
