AccendoWave - A Pain #Data Company Thirty health systems from across the country have signed onto an 18-month initiative to explore how best to provide quality care for older adults. Orchestrated by the nonprofit Institute for Healthcare Improvement (IHI) and the John A. Hartford Foundation philanthropy, the Age-Friendly System-Wide Spread Collaborative will see participating organizations sharing data and other learnings to scale equitable care delivery for the growing demographic. A Top 4 Global Health Equity Solution and Top 15 Global Remote Monitoring Company, AccendoWave, benchmarks objective brain wave pain data (specialty, gender, age) and has nine #pain databases: Emergency Department, Maternal Health, Oncology, MSK, Medical Surgical, ICU, hashtag #Women, Adults, Seniors to eliminate bias, improve outcomes and reduce health care costs. If desired, AccendoWave can also create customized pain databases for partners that can be accessed on the Datavant platform.

Patient Medication Adherence. This issue is estimated to cost $100-$300 billion and result in 125 000 deaths every year. This is not a new challenge—apparently it appears on Hippocrates’ ancient scrolls. But it’s as serious an issue as ever, 𝐞𝐬𝐩𝐞𝐜𝐢𝐚𝐥𝐥𝐲 𝐢𝐧 𝐭𝐡𝐞 𝐚𝐠𝐢𝐧𝐠 𝐩𝐨𝐩𝐮𝐥𝐚𝐭𝐢𝐨𝐧. Patient adherence to medication regimens is estimated around 50-60%, and there is a huge drop-off in adherence after the first month of a new medication or treatment regimen. #aging #seniorcare #adherence #concordance

Patient-centered care is a cornerstone in healthcare, but its implementation poses challenges. As an OT, I recognize the importance of assessing individuals in their own environment to truly understand their functional capabilities. Research shows that Health transitions in older adults depend on effective self-management, and health care providers need to provide care that promotes sustained and effective self-management in older adults rather than promoting patient dependence. A key challenge in transitional care is the need to provide patients with medical care that is adapted to independent, self-directed needs, i.e., their own perceived, rather than professional-defined needs (Older persons' experiences of adapting to daily life at home after hospital discharge: a qualitative metasummary, Hestevik CH, Molin M, Debesay J, Bergland A, Bye A.). 🔍 Exploring the latest insights on transitioning to long-term care, Tate et al's (2023) scoping review highlights key risk factors such as cognitive impairment, functional limitations, and caregiver burden. Excitingly, three of these factors are modifiable, offering hope for proactive interventions. This is good news. We need to end systemic racism to improve all people’s lives, and not just in healthcare. Racism remains rampant in the USA. “A coalition of 12 Philadelphia-area health systems announced Monday that its members have abandoned the use of race adjustments in four clinical tools commonly used to guide care, a move that health leaders say will improve treatment and prevent delays in diagnosis for Black, Hispanic, and Asian patients. The decision marks one of the largest and broadest efforts to date to remove race from widely used clinical algorithms. The inclusion of race in dozens of these tools has sparked vigorous debates and roiled medicine since 2020, when many physicians began pointing out that the tools rely on faulty science that depicts race as biological, and that they can harm some already marginalized patients by making it seem they are healthier than they really are. The health systems involved include most of the region’s major health systems, and they are already seeing positive results from the changes, Ross said. For example, the coalition’s adoption of a race-free equation to calculate kidney disease severity has resulted in 721 patients moving either onto or up the kidney transplant list. In 2023, 63 of those patients received new kidneys. The Philadelphia coalition is the second to take such action. A coalition in New York called CERCA, or the Coalition to End Racism in Clinical Algorithms, has reported that seven of its nine members have deimplemented or addressed the use of race in at least one algorithm. That coalition is led by Michelle Morse, who is currently serving as New York City’s interim health commissioner and has long been a leader in the effort to remove race from algorithms.” #racism #systemicracism #healthcare #commonsense

A 2024 study published in the Journal of Geriatric Medicine highlights the critical challenges and opportunities in providing effective primary care to older adults. The study underscores the need for:  • Enhanced training for primary care physicians in geriatric care.  • Improved coordination of care to manage chronic conditions.  • Increased support for caregivers.  • Adoption of innovative healthcare technologies to monitor and support aging patients.
