Despite the success of primary care #VBC models, specialty services struggle due to #financialdisparities and lack of upfront support. In addition, when we pay for units of care (a.k.a. #FFS) we wind up with disjointed, #disconnectedcare for those who most need a #wholepersoncare to achieve the best #outcomes. Specialists often face financial setbacks in VBC arrangements, as opposed to #primarycare physicians who receive upfront assistance. This hampers the adoption of specialty #VBC models, especially in managing #chronicconditions where specialist #intervention is crucial. To address these challenges and make strides in managing #chronicconditions, there needs to be a #culturechange that includes working with #specialtycare providers, #payers, utilizing #innovations in #technology and #medicine, and considering #nonclinical factors (#SDOH) that impact patients’ treatment and ability to get to the best #outcomes.
