Crowding is a serious problem in EDs, that brings about greater length of stay and greater costs to hospitals. Crowding decreases patient satisfaction and increases medical errors. There are options to address crowding; that can be classified as ED solutions and hospital solutions. Hospital solutions could be tailored to smoothing admissions, early inpatient discharge and full capacity protocols. Some ED solutions are listed below: - Triage solutions: such as bypassing triage when beds available, bedside registration, moving nonemergent patients to a fast-track unit - Scribes: eliminate the wasting time spent on writings - Providers in triage: treating some nonemergent patients in the triage without using a bed - Completion of workup on inpatient unit and bypass a specialty workup in the ED -Triage out: after a medical screening examination, recommending that patients without an emergency condition seek care elsewhere. - Smoothing metrics for better patient flow (reference: Strauss & Mayer’s ED management) #EDcrowding #EDmanagement

Lengthy discharge delays are clogging up acute hospitals because there is often a lack of domiciliary care to cater for discharged patients. But this isn't the only factor keeping beds full, although it's admittedly a significant one. Another reason is that patient care is just not coordinated well enough throughout the hospital. Knowing what's happening now, what's happening next and who's responsible are just three critical factors that enable patient care to be streamlined and, even more important, effective and safe. In fact, CAREFUL, does this and so much more, that length of stay times have been seen to improve notably in hospitals using CAREFUL. Ask us how we do it!

There are a number of arms to this issue. Undoubtedly we require cultures and workload management structures that support collegiality, inter-professional collaboration, and institutional challenge and accountability that leads to the kind of change that optimizes excellence in patient care. The second, and not to be considered secondary in priority to the former, requires a concerted understanding and integration of the ‘experience’ of professional role transition for healthcare professionals. The experience of transition significantly influences and impacts on the new professionals assimilation into their new work context.
