Mechanical Thrombectomy for Acute Ischemic Stroke Associated With an Ipsilateral Internal Carotid Artery Web Carotid web, a shelf-like projection located in the carotid bulb's posterolateral wall, can induce flow turbulence and stasis, promoting clot formation that may cause ischemic stroke. This case report highlights the clinical trajectory of a man who presented with acute right hemiplegia due to left middle cerebral artery occlusion. https://okt.to/nltdgp

Challenging Ischemic Stroke Guidelines Early reduction of blood pressure has a beneficial effect in hemorrhagic stroke but a detrimental effect in ischemic stroke, INTERACT4 data shows. The findings could shake up recommendations on control of blood pressure in acute stroke patients. Acute ischemic stroke, caused by carotid artery dissection, is not common. I recently encountered two such cases. One of which was treated with carotid stenting followed by mechanical thrombectomy, while the other was treated with carotid stenting alone. #HybridVascularNeurosurgeon #NeuroInterventioninUAE #CaritidArteryDissection #AcuteIschemicStroke

Pleased to discuss what is new in lipidology at the Annual ABCD conference today. We discussed issues relevant to diabetologists such as treatment targets, triglycerides and cardiovascular risk and Lp(a). We briefly covered European Society of Cardiology /European Atherosclerosis Society 2019 recommendations of primary and secondary prevention of ASCVD, REDUCE-IT trial and place of icosapent Ethyl in cardiovascular risk reduction. We also covered non-statin medications such as Bempedoic acid and Inclisiran. We discussed Lp(a) as an important risk factor for cardiovascular disease. Everyone with an interest in cardiovascular risk reduction should know about this. We are fortunate to be involved in a number of clinical trials of agents targeting Lp(a). It is always refreshing to have an engaged audience with an interest in making a difference. The future is promising. #ABCD #Lp(a) #cardiometabolic

In my opinion, we are looking at „meningoencephalitis of unknown origin (MUO)“ too one-sidedly. There are many facets that could play a role in both the classification and therapy, we just need to look more closely. We should think about considering not only breed or age but also immunological features. Read our new review https://lnkd.in/eRBqcm_B Let’s discuss 🤓

Great read on Beta-Blockers The data on beta-blockers are decades old. In the time before stents, statins, angiotensin-converting enzyme inhibitors, and dual antiplatelet therapy, when patients either died or got better on their own, beta-blockers showed major benefits. Studies like the Norwegian Multicenter Study Group, the BHAT trial, and the ISIS-1 trial proved the benefits of beta blockade. These studies date back to the 1980s, when you could call a study ISIS without controversy. It was a simpler time, when all you had to worry about was the Cold War, apartheid, and the global AIDS pandemic. It was a time when doctors smoked in their offices, and patients had bigger infarcts that caused large scars and systolic dysfunction. That world is no longer our world, except for the war, the global pandemic, and the out-of-control gas prices.

WOW ... From the 2023 ACC Expert Consensus Decision Pathway on Comprehensive Multidisciplinary Care for the Patient With Cardiac Amyloidosis. "There are over 130 known TTR variants associated with ATTR amyloidosis of which the most common in the United States is the substitution of isoleucine for valine at position 122 of the protein sequence (reported as pV142I or Val122Ile or V122I). The prevalence of the Val122Ile variant has been reproducibly shown to be 3.5% in the self-identified U.S. Black population, corresponding to approximately 1.5 million carriers." https://lnkd.in/enYUW2XN

💡There is still a lot to learn and gaps in knowledge remain more than 25 years after the EU market authorization of #clopidogrel in July 1998, which followed the results of #CAPRIE in The Lancet (https://lnkd.in/dnzy9zQY) ✍️To tie in here, see the excellent summary on „The year in cardiovascular medicine 2023: the top 10 papers in thrombosis and antithrombotic treatment“ by Felicita Andreotti, Michelle O'Donoghue, MD MPH and Jur ten Berg …including studies on #SAPT (aspirin vs. clopidogrel), shortened #DAPT and our meta-analysis on guided vs. potent P2Y12 inhibitor therapy for #ACS & #PCI in European Heart Journal (https://lnkd.in/dp_Y-UWq) by Mattia Galli , Davide Capodanno , Giuseppe Biondi Zoccai , Roxana Mehran , C.
