The U.S. healthcare system faces persistent challenges—high costs, poor outcomes, and inequities—despite numerous improvement initiatives. To address these systemic issues, traditional point-by-point fixes are no longer enough. Enter systems thinking: a comprehensive approach that considers the interconnected drivers of healthcare, from payment models to patient education, and aims to design systems that deliver better outcomes at lower costs. For example, tackling heart failure hospitalization requires more than clinical care; it demands a multidisciplinary, systems-level model that integrates psychosocial care, medication management, and patient education. By analyzing interconnections and designing workflows to optimize outcomes, systems thinking can reduce disparities and build sustainable, equitable care. How can healthcare organizations embed systems thinking into their strategies? Could this approach unlock scalable solutions for chronic disease and health inequities? #SystemsThinking #HealthcareInnovation #PopulationHealth #HealthEquity #ValueBasedCare #PatientOutcomes #HealthcareTransformation #ChronicDiseaseManagement #HealthSystems #MultidisciplinaryCare

A new study by Browne et al. (2024) introduces game-changing tools to assess patient experiences through an equity lens. Here's why it matters: 📊 Two innovative scales developed: Equity-Oriented Health Care Scale—Ongoing (12 items) Equity-Oriented Health Care Scale—Episodic (9 items) 🔍 Key features: • Captures experiences of marginalized populations • Uses an intersectional approach • Proven reliability and validity 💡 Why it's important: • Helps identify systemic barriers in healthcare • Enables targeted improvements in care quality • Promotes inclusive, responsive healthcare for all This research empowers us to create a healthcare system where every patient feels valued and understood. Let's champion these tools to drive meaningful change in health equity! #HealthEquity #PatientExperience #HealthcareInnovation Reference: Browne, A. J., Varcoe, C., Ford-Gilboe, M., Wathen, C. The Future of Equitable, Efficient Population Health Management - HIT Consultant The future of equitable and efficient population health management hinges on adopting a systems thinking approach. Traditional methods often fail to address systemic issues like chronic disease and healthcare disparities. By engaging all stakeholders, analyzing interconnections, and designing targeted changes, healthcare organizations can improve patient outcomes while reducing costs. For instance, comprehensive care models for heart failure demonstrate how multidisciplinary interventions can effectively lower hospitalization rates. To drive meaningful change, we must focus on optimizing healthcare systems rather than just individual behaviors. Embracing this holistic perspective is crucial for addressing the complexities of today’s healthcare landscape. #PopulationHealth #HealthcareIT #SystemsThinking #PatientOutcomes #HealthEquity #ChronicCare #HealthcareInnovation ai.mediformatica.com #health #healthcare #outcomes #design #clinical #hollyurban #patients #busine #data #effectivene #highlighted #management #digitalhealth #healthit #healthtech #healthcaretechnology @MediFormatica (https://buff.ly/4io4ihb)

I am asked quite often my "Why, Intermountain and SelectHealth?". I am proud to work for such an amazing HealthCare system, and help people live their healthiest lives possible. I wanted to share the below article where, Intermountain Ranked #1 in Top Large Health Systems nationally: https://lnkd.in/gtSF3fXh Some highlights are below, Study of 355 health systems across the U.S. using publicly available Medicare data. Considered eight performance indicators, like inpatient mortality and average length of stay, that measure an organization’s clinical outcomes, operational efficiency, and patient experience. If all health systems performed as their top-rated peers, the analysis found, there would be 220,000 fewer patient deaths and 196,000 fewer patients to suffer complications during their hospital stay.

In healthcare, trust between patients and clinicians is often treated as a perk, but it's so much more than that—it's a necessary tool for achieving optimal outcomes. Studies have shown that patients who trust their providers are more likely to follow treatment plans and engage in proactive health management. Building this trust is key to addressing barriers tied to systemic issues like implicit bias. By prioritizing trust, healthcare providers can drive equitable outcomes, reduce disparities, and build meaningful patient relationships. #PatientExperience #Healthcare #PatientCare

At the heart of every successful ACO are dedicated care managers who work tirelessly to address patients' social needs. However, the reality is that their caseloads can often become overwhelming, making it crucial for ACOs to implement strategies to keep them focused and efficient. Here are three key strategies that can help ease this burden and ensure that care teams can continue to provide high-quality care to their patients ➡️ Targeted Outreach Lists: Generate targeted outreach lists to save time and optimize care teams' workflows, ensuring they focus on the right patients. ➡️ Prioritize Interventions: Use data-driven insights to prioritize interventions, eliminating guesswork and ensuring patients are connected to essential social care services. ➡️ Leverage Technology: Implement technology solutions that streamline social care referrals, enhance communication, and provide data to support decision-making, ensuring timely access to essential services. When care teams are empowered with the right technology, focused on the right patients, and understand the impact of interventions on patient outcomes, they can deliver more personalized and effective care. This leads to improved patient satisfaction, better health outcomes, and more efficient use of resources. https://hubs.la/Q02KrrBl0 #accountablecare #sdoh #caremanagement #socialcare

Healthcare is a universal human need, yet not all patients experience it equally, with a patient's needs differing from individual to individual. Minority populations often face the biggest challenges in finding reliable providers who can address these unique needs. According to recent research, patients receive better access to care, feel more positive about their treatment, and achieve better health outcomes when, #healthcare workers reflect the diversity of their community. But how can providers gain a better understanding on the needs and communities of their patients? Well, predictive data is certainly playing a roll! By leveraging advanced data and analytics, healthcare systems can gain deeper insights into healthcare providers' personal characteristics, such as race and ethnicity. This approach empowers the healthcare industry to enhance provider diversity, leading to more personalized care, better patient engagement, and improved health equity.
