Dr. Abhishek Shukla Aastha Hospice - In managing an elderly patient with non-palpable BP, pulse, and low oxygen saturation, prioritize ABC: maintain airway, keep oxygen saturation >90%, and maintain systolic BP >90 mmHg using IV fluids and vasopressors. Apply a medical hypothermia blanket if needed. Investigate the cause by sending cultures, blood gases, and relevant labs. Start broad-spectrum antibiotics. Reassess in 30 minutes for improvement. Though the meme brings a touch of humor, the message here is critical. Antimicrobial resistance (AMR) is a serious global issue that affects many areas of healthcare, especially wound care. Overuse is the frequency and extent of antibiotic use beyond the necessary, while misuse references the incorrect application of antibiotics, including inappropriate prescribing and patient behaviour. Both practices are detrimental and contribute significantly to the problem of antibiotic and antimicrobial resistance. And unfortunately this isn't widely known, nor is the devastating impact it can have. Antimicrobial resistance is emerging as a global health crisis, with superbugs threatening to make routine medical procedures life-threatening, warns Dame Sally Davies, former chief medical officer for England. Currently, AMR claims around a million lives annually, a figure projected to double within 25 years, potentially causing 40 million deaths by 2050. Elderly populations are particularly vulnerable, with AMR-related mortality in over-70s rising by 80% since 1990. The overuse of antibiotics in farming—accounting for 70% of global usage—accelerates resistance, creating superbugs that spread via water, soil, and air. Meanwhile, the pharmaceutical industry lacks incentives to develop new antibiotics due to limited profitability.
