Learning from major accidents is a continuous process of sharing and applying the lessons learned from the investigation, analysis, and improvement of safety incidents. The main purpose of learning from accidents is to foster a culture of safety and to prevent their recurrence. The learnings should be communicated, documented, and disseminated, to all relevant stakeholders, such as employees, contractors, suppliers etc. The learnings should also be integrated, updated, and aligned, with the existing safety policies, procedures, and practices.
