The National Association for Home Care & Hospice and the National Hospice and Palliative Care Organization have come together to form one, new national organization that will carry forward the work of serving home care, home health, hospice, and palliative care providers as they deliver patients the highest-quality care in their own homes and communities. The National Alliance for Care at Home (the Alliance) also launched its integration website today: AllianceForCareAtHome.org. In addition to providing information on the Alliance, this new website serves as a single sign-on hub, enabling members of either legacy organization to access the resources of both. This week, I was reminded that perception, especially a caregiver’s, is tinctured with complex emotions. Several healthcare professionals had informed the caregiver, “There is nothing left to do but hospice.” The caregiver voiced that he understood where their loved one was at in the life cycle yet kept asking for aggressive measures. He was struggling with the decision he was making regarding the direction of the patient’s care. What and how terminality was presented by the interacting care team was a big factor in the caregivers' understanding and perception. These are not one-and-done conversations. Dialogue has to be fluid and ongoing, exploring gaps and doing the best to help guide and foster shared decision-making. This is an excellent read for anyone involved in patient care, especially hospice and palliative care, as it fosters a deeper understanding of the difficult decisions and profound responsibilities accompanying the end of life. #hospice #hospicecare #endoflifecare #palliative #pallitivecare #palliativemedicine #hapc #hpm #nhpco #nach #palliatievezorg #cuidadospaliativos #soinspalliatifs

Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning.
