Medscape has just featured interim results from the #CHANCE study in a recent article. The CHANCE study aims to compare 1-year graft and patient survival rates post-#livertransplantation in patients with acute-on-chronic liver failure #ACLF grade 2 or 3 versus those with #decompensatedcirrhosis without severe ACLF. The study also evaluates transplant-free survival of patients with ACLF-2 or -3 who are not listed for liver transplantation. Coming from the article: 1. DCD Donors in the United States: Donors after circulatory death (DCD) make up more than 30% of the deceased donor pool in the United States. However, organs from these donors tend to have lower transplantation success rates compared to those from brain-dead donors. 2. Cost of Acquiring DCD Livers: The total cost of acquiring a DCD liver for transplantation is $15,029 higher than that of a liver from a brain-dead donor. This cost difference is partly due to 18% of DCD transplantation expenses being attributed to dry runs, where organs cannot be utilized. 3. Impact on Total Costs: DCD transplants account for 34.5% of the total organ acquisition costs, despite representing only 30.3% of the transplantation volume. 4. Strategies to Reduce Costs: To mitigate the costs associated with DCD, strategies such as local organ recovery, transferring donors to nearby hospitals, and conducting more thorough prerecovery organ assessments are recommended. 5. Importance of Expanding DCD: Expanding the use of DCD is crucial for increasing the availability of liver grafts for transplantation. Regarding points 4 and 5, and in my opinion, according to the published evidence, integrating liver perfusion machines into these strategies can significantly enhance the viability of DCD organs, reduce the incidence of dry runs, and improve overall cost-effectiveness; addressing the cost issues, mainly through the use of perfusion technologies, is essential to prevent reversing the progress made in expanding the donor pool and to support the long-term sustainability of DCD transplantation. 📰 We are thrilled to share our latest publication: "New insights in immunomodulation for intestinal transplantation." 📌 Achieving tolerance in solid organ transplantation remains a significant challenge. This is especially true for intestinal grafts due to the small bowel's high immunogenicity. This review highlights the foundational chimerism theory introduced by Thomas Starzl, provides an overview of our current understanding of tolerance in intestinal transplantation, and outlines recent advances in the field. To illustrate, we also focus on the Leuven immunomodulatory protocol, a pro-tolerogenic approach initiated in 2000 which includes several interventions designed to enhance graft acceptance. 👊 Special thanks to Professor Laurens Ceulemans for his tremendous mentorship and unwavering support, and to Professor Jacques Pirenne for his crucial guidance.
