Customized Freeform Scleral Lenses for Enhanced Comfort and Vision in Complex Keratoplasty Case A female patient, born in 1973, was referred to our contact lens institute for a new fitting of custom freeform scleral lenses in both eyes (OU) to improve comfort and extend wear time. She has a complex ocular history with irregular astigmatism due to keratoconus and has undergone multiple procedures: an initial penetrating keratoplasty (PKP) in both eyes in 1999, followed by re-keratoplasty in 2005. In 2011, the right eye (OD) required corneal cross-linking, and later that year, an amniotic membrane transplant was necessary due to bacterial keratitis. Her left eye (OS) developed herpetic keratitis during this period, and in 2012, she received a posterior chamber lens implant in her right eye, followed by another PKP. To improve the fit and comfort of her lenses, we customized i-Shape scleral lenses using data from the Eye Surface Profiler (ESP) by Eaglet Eye. The right lens was fitted with a prolate decentered clearance and 36 sections with 72 radials, while the left lens was configured with an oblate decentered clearance and 36 sections with 72 radials. The accompanying video demonstrates the 3D visualization of both lenses on the model of the anterior eye segment (cornea + sclera) as well as the final fitting results. PTERYGIUM is a raised, fleshy growth on your eye’s conjunctiva, the clear membrane that covers the white part of your eye. The growth may look whiteish or pinkish and may have visible blood vessels in it. It typically starts in the corner of your eye and grows toward iris. Pterygium typically has a triangular or wing-like shape. The word, “pterygium,” comes from the Greek words pteryx, meaning “wing,” and pterygion, meaning “fin.” Another name for pterygium is surfer’s eye. This is because surfers are often in the elements that cause pterygium — sun, wind, sand and salt. Surfers do get pterygia (the plural of pterygium) more often. But anyone can get one if they spend a lot of time outdoors without eye protection. It’s more common in older adults, who’ve had more exposure to the elements over time, and also in those who live near the equator, where sunlight is more intense. A pterygium isn’t an invasive growth. It’s not cancerous, and it won’t spread to other nearby tissues. But it can continue to grow across the surface of your eye. If it reaches your cornea (the clear part that covers the colored part), it can damage your vision. In this case, you might need surgery to remove it. Tx: Conservative Management includes Lubricating Eye Drops, Topical Steroids/NSAIDs and protection from UV radiation. Surgical Excision: Indicated in cases perhaps such as this, where pterygium encroaches on the visual axis, affecting vision.

An online search was performed for articles published in PubMed and EMBASE between January 1, 1975, and January 7, 2023. Original studies reporting postoperative complications of sutureless intrascleral IOL fixation were included.
