Dear Friends,
The clock always seems to tick louder and faster in December as we anticipate the arrival of the 12 days of Christmas, the eight nights of Hanukkah, the seven days of Kwanzaa, and then the New Year’s Eve countdown of the final minutes of the year.
FPHNYC will begin 2025 managing more than 100 projects in partnership with the NYC Health Department and community-based organizations throughout the city. And we will continue to work to realize the goal of HealthyNYC to increase New Yorker’s life expectancy and reduce health inequities.
This busy agenda of public health work builds upon all we accomplished this year – accomplished through collaboration with and the generosity of our partners in philanthropy, the private sector, and many individuals. With your continued support, we look forward to another year of making a positive and measurable impact on the health and well-being of all New Yorkers.

FPHNYC Encourages Philanthropy Leaders to Address Chronic Diseases
Chronic diseases, such as diabetes, heart disease, and screenable cancers, are the number one cause of death for all New Yorkers. FPHNYC and the NYC Health Department recently held a roundtable for foundations and other philanthropic funders to preview a forthcoming plan to address chronic diseases and reduce health inequities.
Public-private partnership will be essential to help realize many components of the plan and see them scaled up to meet the needs of New Yorkers. In the coming months, FPHNYC will be organizing discussions with subject matter experts exploring many of the Chronic Disease Plan’s specific proposals and opportunities for cross-sector collaboration.

W.K. Kellogg Foundation Supports Maternal Home Collaborative
The innovative Maternal Home Collaborative (MHC) to reduce Black maternal mortality and morbidity in NYC has received inaugural funding from the W.K. Kellogg Foundation. The MHC, a program of the NYC Health Department created in collaboration with community and health provider stakeholders, will coordinate clinical care, services at brick-and-mortar community spaces, and a variety of social support (e.g. peer-to-peer groups, counseling, home visits), to help pregnant New Yorkers and their families.
The W.K. Kellogg Foundation is providing $600,000 over three years to support the planning and implementation of the MHC in under-resourced neighborhoods in Brooklyn. This investment is critical to addressing a public health crisis: Black New Yorkers are more likely than white New Yorkers to experience a pregnancy-associated death. Yet, most of these pregnancy-associated deaths are preventable through enhanced maternal health programs and services like those brought together by the MHC.
As part of Healthy NYC, the City has set a goal of reducing Black maternal mortality by 10 percent by 2030. The MHC is positioned to make meaningful strides toward improved maternal health outcomes and health equity.
