Developing a Framework for TQM Implementation in a Healthcare

Maintaining high quality standards is very important for health care facilities. 

During my participation, as Chief Engineer and Quality Manager at luxury Brands in hospitality industry, in a case study research focusing on the PeopleCERT Accessibility Pass training and certification scheme as best practice, for the purposes of the EU Study: Mapping the Skills and Training Needs to Improve Accessibility in Tourism Services (Award by the European Commission DG Enterprise, Tourism and Cultural Instruments Unit 2013-2014), I stated the following : The Total Quality Management system is a process required to prove the level of services offered, and at the same time identify points of non-compliance, so as to improve quality through relevant modifications. “There is no room for mistakes in the hospitality & in the Healthcare Industry”. It is a process required to prove level and quality of services offered, and at the same time identify points of non-compliance so as to improve quality through relevant modifications ... ».

The term quality of patient care is still subject to debate and has no specific definition yet, since the guides to quality of healthcare are highly probabilistic due to estimations of the outcomes. The outcomes are not certain; therefore the measures of its quality must include dimensions of both the outcome of the care provided and processes by which the care is carried out. The measures of quality of care should include an evaluation of the provider-patient relationship.

In TQM, systems are established to prevent clinical and administrative problems, increase patient satisfaction, continuously improve the organization’s processes, and provide healthcare services as better, then those of the competitors.

Customer focus, error prevention, employee participation, teamwork, systemization, leadership and continuous quality improvement are TQM universal management concepts that can be transferred to any business setting. TQM uses quality as the fundamental measurement metric, continuous improvement as the philosophy and employee involvement as the approach.

The quality is assessed in terms of Quality of Technical Care, Quality of the art of Care, Quality of the Environment.

Minimum infrastructure required for making available a particular service. Clarity regarding the Buildings, the Space, the Equipment, the Environment and the Staff.

Deming, in his model, defined TQM as a method of leadership and management which:

• Analyzes system for errors and variation rather than blaming people

• Develops long term partnership with external and internal suppliers

• Uses accurate data to analyze processes and measure system improvement

• Involves the staff who work in system analysis and improvement

• Sets up effective collaborative meetings as the basis of teamwork

• Train supervisors and managers in leading the ongoing improvement process

• Engages staff in setting targets and ensures that results are feedback

• Highlights the need for senior executives to plan strategically

• Achieves long-term improvement through small incremental steps

Total quality management implementation ensures that the employees and management jointly engage in the production of goods and services. The integration of quality management and quality goods and services promotes increase in the organizational opportunities and reduces chances of failure due to minimum utilization of the available resources.

The healthcare facilities should appoint an office to be responsible for ensuring quality of care in hospitals, ambulatory surgical treatment centers and nursing homes. 

It is important to maintain quality because if the customers’ needs are not satisfied, they may share their opinions and critism through the social networking sites like twitter and Facebook which may damage the organization’s reputation and publicity.

Accreditation enables the health care to gain new customers by providing confirmation to their prospective customers of the medical facility’s ability to provide quality services.
