This week, we honor the hard work of our clinicians, support staff and leadership who make this vision a reality every day.

At Enhabit, quality isn’t just a goal—it’s embedded in our culture. One of the key ways we enhance the care experience is through Age-Friendly Care. By prioritizing the specific needs of older adults, we ensure that our practices positively impact their lives, leading to improved outcomes and a better patient and family experience. This approach means listening closely to what matters most to our patients and their families, and tailoring our care to support their preferences, routines and health care goals.
Commitment to continuous improvement
In home health and hospice, continuous improvement is essential. We have a strong process for evaluating care delivery using root cause analysis to identify areas for growth. This helps us adapt as the health care landscape evolves, always keeping the patient’s needs at the center. By aligning with regulatory, risk and billing standards, we remain flexible while maintaining the highest level of care.
Incorporating patient and family feedback
Patient and family feedback is key to our quality improvement efforts. We use our Age-Friendly Care framework, focusing on the 4Ms—What Matters, Medication, Mentation and Mobility—to address their needs. By analyzing survey results, we continually refine our approach to enhance the experience for patients and their caregivers, ensuring we’re meeting their needs and expectations.
