Understanding Anchoring Bias in Medical AI

Anchoring bias is a cognitive bias that describes the common human tendency to rely too heavily, or "anchor," on one piece of information—usually the first piece of information encountered (the "anchor")—when making decisions.

In the context of healthcare AI, anchoring bias can manifest in several ways:

  1. Model Development: If developers rely too heavily on a specific dataset or feature while building a model, the AI could become overly biased towards that data, potentially leading to less accurate predictions when confronted with new or different data. For instance, if an AI system is primarily trained on medical images from a certain demographic, it might perform poorly when used on a different demographic.
  2. AI-assisted Diagnosis: If a physician receives a diagnostic suggestion from an AI tool early in the diagnostic process, they might become anchored to that suggestion even if subsequent evidence suggests another diagnosis.
  3. AI Interpretation: When radiologists, for example, use AI for interpreting medical images, the first result or highlight presented by the AI might influence the subsequent review, even if the AI's initial highlight was not the most pertinent or accurate.
  4. Decision Support Systems: In cases where an AI provides treatment recommendations, if the first option is always a certain drug or procedure due to the way the algorithm is designed, it could unduly influence the healthcare provider's choices, leading to suboptimal treatment decisions.

To mitigate the effects of anchoring bias in healthcare AI:

  1. Diverse Training Data: Ensure the AI model is trained on a diverse and representative dataset to minimize biases.
  2. Blind Review: In situations where AI assists in diagnoses, consider having professionals review some cases without AI input initially, and then with it, to compare results.
  3. Continuous Feedback Loops: Implement systems where professionals can provide feedback on AI recommendations, and use this feedback to continuously refine the system.
  4. Education and Training: Healthcare professionals should be made aware of potential biases, including anchoring bias, when using AI tools.

In essence, while AI has the potential to greatly assist in healthcare, it's essential to be cognizant of biases like anchoring, so that these tools augment, rather than hinder, the decision-making process.

Anchoring bias in established ML/AL systems seems to be a problem that is both tractable and fixable. It would involve retraining models with diverse datasets, rather than simply appending diverse data onto pre-existing training datasets (which were not vetted for diversity).
