CMS CY 2025 Final Payment Rule 

Finalized Physician Payment Rule

On November 1, 2024, Health & Human Services announced finalized new policies for Calendar Year 2025 Medicare Physician Fee Schedule (PFS). This includes increased access to services and builds upon policies established in previous years.

Unfortunately, average payment rates under the PFS will be reduced by 2.93% in CY 2025, compared to the average amount these services were paid for most of CY 2024. 

Office/Outpatient (O/O) Evaluation and Management (E/M) Visits - For CY 2025, we are finalizing our proposal to allow payment of the O/O E/M visit complexity add-on code, Healthcare Common Procedure Coding System (HCPCS) code G2211, when the O/O E/M base code — Current Procedural Terminology (CPT) codes 99202-99205, 99211-99215 — is reported by the same practitioner on the same day as an annual wellness visit (AWV), vaccine administration, or any Medicare Part B preventive service, including the Initial Preventive Physical Examination (IPPE), furnished in the office or outpatient setting.

Hospital Outpatient Prospective Payment (NFRM)

Notice of Final Rulemaking Regulation CMS-1809-FC

The Centers for Medicare & Medicaid Services (CMS) announced today new baseline health and safety requirements for hospitals and Critical Access Hospitals (CAHs) providing obstetrical (OB) services to make pregnancy, childbirth, and postpartum care safer. CMS is also removing barriers to expand access to care for those formerly incarcerated and others in underserved communities.

These policies are included in the calendar year (CY) 2025 Hospital Outpatient Prospective Payment System (OPPS) and Ambulatory Surgical Center (ASC) final rule released November 1, 2024, which also updates payment rates for hospital outpatient and ASC services for CY 2025 by 2.9% (3.4% market basket, less 0.5 percentage points for multifactor productivity). These payment updates are estimated to result in an additional $2.2 billion in CY 2025 OPPS payments for hospitals compared to CY 2024. ESRD Prospective Payment System

End Stage Renal Disease PPS Final Rule CMS-1805-F

The ESRD PPS provides a bundled, per-treatment payment to ESRD facilities.  The bundled payment includes all renal dialysis services furnished for outpatient maintenance dialysis, including drugs and biological products and includes all other renal dialysis items and services that were formerly separately payable under previous payment methodologies. The bundled payment rate is case mix adjusted for a number of factors relating to patient characteristics.

For CY 2025, CMS is increasing the ESRD PPS base rate to $273.82, which CMS expects will increase total payments to all ESRD facilities, both freestanding and hospital-based, by approximately 2.7%. The CY 2025 ESRD PPS final rule also includes changes to the methodology for calculating the ESRD facility wage index, changes to the Low-Volume Payment Adjustment (LVPA) methodology, and several changes to the ESRD outlier policy.  Learn more – access the November 1, 2024 Fact Sheet  or access the Federal Register posting.

CY 2025 Home Health PPS

Home Health Prospective Payment System Final Rule CMS-1803-F

The HH PPS Final Rule updates Medicare payment policies and rates for Home Health Agencies (HHAs). This rule finalizes a permanent prospective adjustment of -1.975% (half of the calculated permanent adjustment of -3.95%) to the CY 2025 home health payment rate to account for the impact of implementing the Patient-Driven Groupings Model (PDGM).

This rule also updates the intravenous immune globulin (IVIG) items and services’ payment rate for CY 2025 for Durable Medical Equipment (DME) suppliers. As described further below, CMS estimates that Medicare payments to HHAs in CY 2025 would increase in the aggregate by 0.5%, or $85 million, compared to CY 2024.

OASIS - CMS is finalizing a crosswalk for mapping responses on the current Outcome and Assessment Information Set-E (OASIS-E) to the prior OASIS-D responses for use in the methodology to analyze the difference between assumed and actual behavior changes on estimated aggregate expenditures; recalibrated PDGM case-mix weights; and updated low-utilization payment adjustment (LUPA) thresholds, functional impairment levels, and comorbidity adjustment subgroups.
