Lombard Chronicles - 29th Day (ex-isolation)

The fifth week of isolation begins the observation that when it seems that a new rule has been identified (read: "more tests") reality hastens to disprove you. 

The number of tests fell again to 5,025 and the new positives (probably "derived" from the tests of the previous days) were 1,079. Actually, in the average of the last two days the total number of tests is aligned to the last week around 6,500 daily. 

Our particular insistence on the "testing strategy", as well the need to proceed with much more significant volumes than the current ones, finds every day new authoritative confirmations as, for example, in the document produced by the English Government last April 4.

It is worth using the words of Matt Hancock, Secretary of State for Health and Social Care ""The Government is setting the challenge to itself and to the whole system - industry, academia, the NHS - to immediately scale up our testing levels to 100,000 tests a day from all five pillars of the strategy, across the UK by the end of this month. This is a huge ambition. Currently, we are delivering around 10,000 swab tests per day. And we will continue to scale up our ambition beyond this to 250,000 tests a day from all five pillars. We will work collaboratively with each nation, allocating targets and tests to meet their specific needs."

The UK government therefore aims to scale up to 100,000 tests per day by the end of April and up to 250,000 tests per day thereafter. 

As a reference, we consider that, from last February 24th to today, we have performed 721,732 tests in Italy. So an average of 16,784 tests per day. 

But why is it so important to increase the number of tests? 

There are several very good reasons for this. Let's go in order of priority. 

 1. Securing the "front line" in the fight against the virus 

As the usual "infographic" bulletin from the Istituto Superiore di Sanità informs us, as of yesterday, April 6, of the 124,527 cases of COVID-19* there were 12,681 health workers affected, equal to 10.2% of the total. Incidentally, we point out that the total number of health workers in Italy was 603,775 in 2017 (as documented by the Ministry of Health) and therefore equal to about 1% of the Italian population. It is evident (and painfully trivial) that this is a sub-population overexposed to infection. Moreover, as the family doctors remind us in the open letter sent yesterday to the Lombard health authorities: "the failure to perform swabs on health workers in the area and in some public and private hospital facilities, with further risk of spreading the infection".

2. Tracing and promptly isolating the potential "diffusers" of the virus 

In the 4/5 of Covid-19 patients are asymptomatic, as we have documented several times from this blog and also recently shown in this study of the British Journal of Medicine

Current containment measures will allow us to reduce the volume of infected patients but will not allow us to eradicate it. The virus will remain latent in the population until the vaccine arrives in 12-18 months (and it is not at all imaginable to stay 12-18 months all locked indoors). 

3. Containing the risk of a very serious economic recession

Confindustria estimates for each week of "lockdown" a cost in terms of Gross Domestic Product of 13.5 billion, equal to 0.75% of GDP, and estimates the loss for 2020 to be at least 6%. The International Labour Organization (ILO) estimates the global impact on employment at a loss of 25 million jobs. 

We therefore need an instrument that will allow the progressive "reopening" of the productive system without putting the health of citizens at risk. 

As Tony Blair also reminds us in the document cited at the beginning of this report:"I do not see a way out of this crisis without mass testing and the development of a clear exit strategy involving testing and tracing, together with innovations in treatment and the use of technology".

In our own small way, we are trying, through the #tamponailvirus project, to fund research for the identification of a methodology that will allow us to identify reliable tests and establish appropriate health protocols. But time is running out and it's really a scarce resource. 

So now, let's look at the numbers, anticipating that, here in Lombardy, we have certainly reached the "peak" but that the "dimensions" at stake, are such as to still impose caution and, above all, show the absolute need for a change of pace (and strategy).

The load on the health system has stabilized, now for a week, although at a very high level (about 13,000 hospitalizations). 

The number of "new positives", always keeping in mind the thousand precautions that my four readers now know by heart, is slowly decreasing.

and suggests a possible reduction to end-February/early March levels by the second half of April.

That, as the family doctors have also explained to us, the "positives" are in fact for the most part serious cases, the trend of the two curves of deaths together with the new positives, which appears to be definitely aligned.

The large mass of almost 14,000 hospitalized patients continues to produce, unfortunately, serious cases that result in intensive therapy (+26 yesterday after the drop of the previous two days) and also deaths.

The dynamics of which remains in relative decline, even if always on high values (yesterday +297 cases, up compared to the previous day, but still on a downward trend).

The volume of hospital discharges remains unsatisfactory, a sign of the complexity of hospitalized cases and the average length of stay, which appears to be close to 4 weeks. 

There has been a sharp increase in home admissions/quarantines, which now account for more than half of cases (15,112).

The international picture shows, at this point, only confirmation of the scenario that has been progressively created in recent weeks, with the substantial alignment of the European countries examined with the Italian dynamic and the strong American divergence.

The mortality trend also shows few surprises.

It is always Germany, (which I will tell you about in a next Chronicle) to show a moderate trend and similar to the Chinese one and, as a small indication, without auto-spoil the analysis, the secret is all in the prevention through tests.

The real good news is that, as in some B series science fiction movies, it's the whole humanity that is fighting against this "alien" and the result is already written: #we will win!

Grazie anche per oggi della sua sintesi cosi’ esaustiva e anche seLl Lei non e’ uno statistico/scienziato/dottore la sua esposizione ha una rilevanza molto seria che mette a disposizione dati fonti e ragionamenti di logica. Da troppe persone si sentono notizie e informazioni frammentate spesso farraginose se non peggio. E poi diciamolo e’ spinto da Passione Passione di chiarezza e senso Civico.
