Prominence Powerhouse: Erica Crisologo
Introducing this month's Prominence Powerhouse: Erica Crisologo, our Director of Care Management and Transitions of Care at Prominence Health. Erica's decision to join our team stemmed from a desire for innovation and creativity, fueled by a chance encounter that led her to our organization. In her current role, Erica leads the Care Management and Transitions of Care team, where she focuses on improving outcomes for our SNP membership and preventing hospital readmissions through proactive care strategies. Erica is a true rock star who drives positive change at Prominence Health and inspires those around her every day.
What inspired you to pursue a career in healthcare?
My healthcare journey wasn't initially driven by grand inspiration. It was more circumstantial. However, what has kept me rooted in this field is genuinely meaningful. Over the course of my nursing career and through various nursing roles, I've discovered fulfillment in contributing to someone's quality of life.
Why Prominence Health?
I was previously employed at a Managed Care Organization in Southern Nevada and felt unfilled. Seeking more innovative and creative opportunities, I stumbled upon a social media post from a provider discussing Prominence Health. After reaching out to him, he connected me with Steve Estipona, who informed me about an available position. I’ve been with Prominence for three years now and feel inspired and challenged daily!
Do you have a favorite or impactful member story you can share?
In my previous role as a CM, I had the privilege of assisting a member who had lost her partner. Together, we navigated through her grief by connecting her with a local organization and helping her find a pet for companionship. Even after I transitioned to another position, she continued to reach out, sharing jokes she would tell from comedy night at the local organization she had joined. She even named her dog after me!
Are there any projects or initiatives you're particularly passionate about?
There are so many. Currently, as part of the Care Advance Implementation, my goal is to guide our CM Team towards prioritizing meaningful interactions with our members, focusing on conversations that truly impact their quality of life rather than solely on documentation.

What is your proudest accomplishment so far?
My proudest accomplishments are my two girls: Haley, who turns 5 this month, and Addy, who is 8 months old.
What do you enjoy doing outside of work?
Beyond being a mom, I enjoy off-roading and spending time outdoors with my family. I'm also a huge fan of Korean dramas, which I turn to for relaxation at the end of the day when the kids are asleep. I'll happily sit in silence reading subtitles!
If you could have dinner with any historical figure or fictional character, who would it be and why?
Richard Branson - the man is a genius, an innovator, a great leader, and he has a great heart.
What advice would you give to your younger self?
I would have told my younger self to travel more!
