Thank you for joining the Population Health Roundtable Session at the 2023 APG Annual Spring Conference, hosted by Health Tech for Medicaid (HT4M) and COPE Health Solutions! The event focused on various aspects of population health, including value-based payment strategies, provider engagement & practice transformation, data analytics, and care management, and featured panelists Adimika Arthur, CEO & Executive Director at Health Tech for Medicaid (HT4M), Allen Miller, Principal & CEO at COPE Health Solutions, Christina Mendez, Senior Managing Director at Aon, and Jeff Conklin, Chief Payor Strategy Officer at Oregon Health & Science University.
During this roundtable session we explored the importance of population health for providers and physician groups, emphasizing the shift towards value-based care models, data analytics, and care management to enhance patient outcomes, improve care coordination, and address community health needs.
Panelists highlighted the benefits of leveraging data analytics for personalized care, the role of value-based payment strategies in driving efficiency and patient-centered care, the importance of reinsurance in mitigating financial risks, and the integration of effective care management to achieve better outcomes.
Provider engagement and practice transformation were also emphasized as key factors in improving patient care and driving positive change. The session showcased the transformative potential of population health approaches for healthcare organizations and the communities they serve.
