TPA vs ASO; how are claims processed for your group health plan and does it matter?

Last week I wrote about the the different components of a health plan and the concepts of bundling vs unbundling. Over the next few weeks I'll unpack some of those components.

Whether you are in a ASO or TPA arrangement, the processes these functions perform are the backbone of what makes your health plan work. Like a kitchen staff at a restaurant these are "back of the house" operations that are essential, critical components of your group health plan. Some of the services they handle are:

  • Claims Administration and Processing
  • Health Plan Administration
  • Underwriting
  • Benefit Plan Design
  • Insurance Carrier & Provider Network Negotiation and Coordination
  • Stop Loss Negotiation & Procurement
  • Pharmacy Coverage Negotiation & Procurement

TPA

A TPA (Third Party Administrator) is the entity that works with all your self-funded carriers and vendors.  Their core services are to adjudicate claims, pay providers and provide claims reporting. They also may act as a coordinator for your other benefit providers such as; stop loss carriers, ancillary providers, wellness providers, COBRA administrators, enrollment services, etc. They also usually offer a whole array of other a la cart services.  

ASO

ASO (Administrative Services Only) performs a lot of the same services as a TPA, but they are generally owned or controlled by an insurance carrier. Going back to my post from last week, an ASO provider will typically accompany a “bundled” arrangement. You find an ASO provider most often when you have an insurance carrier manage multiple components of your health plan.

An ASO arrangement can allow for ease of claims processing because the claims administrator is working directly with the insurance carrier that is covering the claims. While there still may be claim lag on the provider end, an ASO provider will generally pride themselves on quick claims processing. Often auto-adjudicating a lot of their claims, meaning that if the claims coming through the system meet certain criteria they are automatically paid.

A word of caution here though. Fast claims payment in the world of medical billing often results in a plan paying costs that are not accurate due to things like billing errors. If you are managing a group health plan you should be prioritizing accuracy over speed as it relates to claims payment!

They're just paying claims, does it really matter?

Yes!

Providers often don't have their house in order when they are billing patients and health plans. I know, huge surprise there! On the small end, these provider bills are riddled with errors, duplicate billing and erroneous charges. On the drastic end, providers have been found to bill fraudulently, utilizing tactics where they bundle charges together and charge for those same services individually and misclassifying costs to serve their own interests. Whether we are talking about errors, waste or fraud doesn't really matter, what matters is that you want claims administrative processes that clearly identify your coverages, provider billing charges and correctly match up the two, only paying for covered charges at the agreed upon rates. Medical billing is a complicated world and you need a provider who has developed equally sophisticated processes to navigate it.

TPA's are touted for their "Independent" nature - why does that matter?

Football season is about to kick off this week (see what I did there)! So lets look at a football analogy. Imagine if the NFL let the players decide the outcome of plays rather than the ref. A ball carrier deciding where the ball should be placed, a player deciding whether or not he committed a penalty, a receiver deciding whether or not he was out of bounds. There would be some honest players that would make accurate decisions but there would also be a ton of funny business going on right?

Now imagine this scenario. An insurance carrier is making profit off of each dollar of medical premium paid by a health plan. This means that in a general sense, more claims year over year means more profit. Insurance carriers would have you believe that is not the case because in a "bad year" they pay out more claims but in turn they raised your rates exponentially right? How about in the "good year?" They returned that favor by granting you a discount right? Wait, no discount? So if the insurance carrier is making more money off of higher claims and they control the ASO then what incentive would they have to ensure that billing errors, fraud and waste are aggressively identified and eliminated? Hmmm??


What should I be looking for in a claims administrator?

  • Is there a conflict of interest? Are they owned by an insurance carrier, a vendor or an advisor? Does their ownership, affiliation or partnership create disincentives for them to serve your best interests?
  • Effective claims adjudication processes. First an foremost a TPA or ASO pays claims. Making sure that their processes identify fraud, waste and billing errors will save your plan $$$$.
  • Services your plan needs. Understanding what additional services your plan needs can help you identify the right type of partner. Do you need ACA Compliance, COBRA administration, Self-Funded plan administration, Network Contract Negotiation, benefit platform/ technology, etc.
  • Complimentary Services. The right administrator will be a value-add to your team. Their services will compliment what you already have with your internal team, advisor and carrier partners. They may offer enrollment services you need or a benefits delivery platform. They may have underwriting or reporting capabilities that can compliment that of your carrier and/ or advisor. They may have negotiated contractual arrangements with network providers, stop loss carriers or PBM's that could fit well with your plan.
  • Good value for your dollar. A good ASO or TPA provider will offer their services at a reasonable price and you should shop price. However, I would caution you to view this very carefully as to not be "penny wise and pound foolish." I good TPA will save 10-15% in fraud, waste and errors in claims processing so saving 5% with a less expensive option to miss out on those additional savings is not the move.


Here's your homework assignment:

  • Look at your current structure to identify if you have an ASO or TPA arrangement.
  • Obtain the contract for your claims administration if you have not already done so.
  • Identify what systems, processes and resources your claims administrator has for identifying duplicate claims, unbundling claims and identifying claim errors.
  • Identify what services you need from your administrator along with other services that could be offered or handled by an administrator.
  • Utilize your advisor to benchmark your claims administration costs.
