What is Trodelvy used for?

Trodelvy (sacituzumab govitecan-hziy) is commonly used to treat certain types of breast cancer or bladder cancer.

How does Trodelvy work (mechanism of action)?

Trodelvy delivers chemotherapy directly to cancer cells, helping to limit damage to healthy cells. Trodelvy targets certain cancer cells by using one of its parts—a monoclonal antibody that finds and sticks to a protein called Trop-2, which is found on many cancer cells. Once it enters the cancer cell, another part of the medicine called SN-38 becomes active and blocks a key enzyme, preventing the cancer cells from growing and eventually dying.

How is Trodelvy supplied (dosage forms)?

Trodelvy is available in an injectable form that is given by a healthcare provider.

How should I store Trodelvy?

Trodelvy is given by a healthcare provider in a hospital or clinic. You will not store it at home.

Side Effects

What are the most common side effects of Trodelvy?

The most common side effects of Trodelvy are listed below. Tell your healthcare provider if you have any of these side effects that bother you.

  • Decreased white blood cell and red blood cell counts 
  • Diarrhea (see below)
  • Nausea and vomiting (see below)
  • Feeling tired or weak 
  • Hair loss  
  • Constipation 
  • Increased sugar levels in the blood 
  • Decreased protein levels (albumin) in the blood 
  • Decreased appetite
  • Changes in kidney function lab test
  • Increased levels of an enzyme in the blood called alkaline phosphatase
  • Decreased levels of magnesium, potassium, and sodium in the blood

What are the serious side effects of Trodelvy?

While less common, the most serious side effects of Trodelvy are described below, along with what to do if they happen.

Severe Allergic Reactions. Trodelvy may cause allergic reactions, which can be serious. Tell your healthcare provider if you develop any of the following when you get Trodelvy or within 24 hours after you get a dose. 

  • Breathing problems or wheezing
  • Racing heart
  • Fever or general ill feeling
  • Swollen lymph nodes
  • Swelling of the face, lips, mouth, tongue, or throat
  • Trouble swallowing or throat tightness
  • Itching, skin rash, or pale red bumps on the skin called hives
  • Nausea or vomiting
  • Dizziness, feeling lightheaded, or fainting
  • Stomach cramps
  • Joint pain

Infusion Reactions. Trodelvy can cause reactions when it is given to you that can be serious. Your infusion may need to be slowed or stopped if you have these reactions. Tell your healthcare provider right away if you have any of the following symptoms when you get your infusion or within 24 hours after getting a dose. 

  • Fever or chills
  • Redness of your face and neck
  • Trouble breathing
  • Dizziness or feeling faint

Nausea and Vomiting. Nausea and vomiting are common with Trodelvy and can be serious. Take any medicine your healthcare provider gives you to help prevent nausea and vomiting. Tell your healthcare provider if you cannot drink water or if you are having symptoms of dehydration, such as lightheadedness, dizziness, or faintness. Call your healthcare provider right away if you have nausea or vomiting that will not stop.

Low Neutrophil Levels (Neutropenia). Trodelvy may cause low levels of a type of white blood cell (called neutrophils), which is known as neutropenia. Neutrophils help prevent infection. Having a low level of neutrophils may increase your risk of infection, including serious infections. Tell your healthcare provider right away if you have any of the following symptoms of low neutrophil levels.

  • Fever, chills, or sweats
  • Tiredness
  • Sore throat, cough, or shortness of breath
  • Mouth or throat sores
  • Swollen lymph nodes
  • Burning or pain when you pee

Diarrhea. Diarrhea is common with Trodelvy. Severe diarrhea can cause you to lose too much water, which can cause kidney problems. Try to drink enough water to stay hydrated. If you are given medicine to prevent or treat diarrhea, take it exactly as your healthcare provider tells you. Call your healthcare provider right away if you have any of the following.

  • First time you get diarrhea during treatment with Trodelvy
  • Diarrhea that does not go away within 1 day
  • Diarrhea with weakness, dizziness, or feeling faint
  • Black or bloody poop

Who should not use Trodelvy?

Allergies to Ingredients. People who are allergic to any of the following should not use Trodelvy.

  • Trodelvy
  • Sacituzumab govitecan-hziy
  • Any of the ingredients in the specific product dispensed

What should I know about Trodelvy before using it?

Trodelvy can cause severe side effects. Keep all appointments to have your blood checked. This medicine may cause serious side effects soon after starting it, especially if you have reduced activity of an enzyme called UGT1A1. Tell your healthcare provider right away if you get an infection, fever, or have any symptoms of anemia such as the following.

  • Unusual weakness or tiredness
  • Cold hands and feet
  • Fast or abnormal heartbeat
  • Pale or yellowish skin
  • Dizziness, lightheadedness, or feeling like you are about to pass out
  • Shortness of breath

Take any medicine your healthcare provider tells you to take before or after getting this medicine.

Trodelvy may affect your ability to become pregnant. Talk with your healthcare provider if you have any concerns.

What should I tell my healthcare provider before using Trodelvy?

Tell your healthcare provider about all of your health conditions and any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. This will help them determine if Trodelvy  is right for you.

In particular, make sure that you discuss any of the following:

Genetic Factors. If you have been told that you carry the UGT1A1*28 gene variation, you may have an increased risk of serious side effects from Trodelvy. Tell your healthcare provider about any genetic results you have before using Trodelvy.  

Liver problems. Trodelvy may not be the right medicine if your liver is not working as well as it should be. If your liver health is a concern, your healthcare provider may perform tests to determine whether it is working well enough to take this medicine.

Other Medicines and Supplements. Trodelvy may interact with other medicines and supplements. Before using Trodelvy], tell your healthcare provider about any prescription or over-the-counter (OTC) medicines, vitamins/minerals, herbal products, and other supplements you are using. See the Interactions section for more details.

Pregnancy. Trodelvy can cause harm to an unborn baby. A negative pregnancy test is needed before starting treatment. Use effective birth control while you are taking Trodelvy and either for 6 months after your last dose if you can become pregnant or for 3 months after your last dose if you have a partner who can become pregnant. Talk to your healthcare provider about your birth control options. If you become pregnant or think you may be pregnant, tell your healthcare provider right away.

Breastfeeding. Do not take Trodelvy while you are breastfeeding and for 1 month after your last dose. It is not known if Trodelvy passes into breast milk. Tell your healthcare provider if you are breastfeeding or plan to breastfeed.

Overdose/Missed Dose

What should I do if I accidentally use too much Trodelvy?

If you or someone else has used too much Trodelvy, get medical help right away, call 911, or contact a Poison Control center at 800-222-1222.

What should I do if I miss a dose of Trodelvy?

Trodelvy is usually given at a hospital or infusion clinic. If you miss a dose, talk to your healthcare provider. They will adjust your dosing schedule accordingly.
