We work in partnership with primary health care providers, health professionals, health system partners and local communities to improve the health and wellbeing of everyone in our region. Our work supports people to access safe, coordinated healthcare and services, where and when they need it. Brisbane South PHN an independent, not-for-profit organisation funded by the Australian Government. Launched ahead of the busy Christmas holiday period when available healthcare services can be limited and many consumers are left unsure where to go, FindCareFast.com.au places the right option at your fingertips via desktop or mobile browser. 🔗 Seamlessly integrated with Healthdirect Australia and 13 HEALTH to support users in deciding where to go for treatment and advice. 🗣️ User-friendly, accessible, plain language and offering language translation functionality to support culturally and linguistically diverse users to navigate the site. 🌓 Featureing an after-hours mode to direct users to available services open outside of business hours. 👣 Includes dedicated healthcare options for First Nations peoples. We recently held our first of 4 Disability Health Forums in partnership with Metro South Health. At our first forum, we discussed ways to: ✅ improve healthcare for people with disability in our region ✅ integrate learnings and recommendations from the Royal Commission into Violence, Abuse, Neglect, and Exploitation of People with Disability ✅ build stronger relationships between people with disability, health workers and disability community leaders. Thank you to the people with lived experience and representatives of disability peak bodies, advocacy organisations and government partners for joining us. Today is World COPD Day. Chronic Obstructive Pulmonary Disease (COPD) is a group of progressive lung conditions which includes emphysema, chronic bronchitis, and chronic asthma. This condition causes the narrowing of the bronchial tubes in the lungs. Around 1 in 13 Australians aged 40 years and over have a form of COPD. COPD Symptoms can include: ✅shortness of breath ✅repetitive cough ✅increased phlegm or mucus ✅fatigue ✅frequent chest infections ✅longer recovery times. The Royal Australian College of General Practitioners (RACGP) has launched their updated Management of type 2 diabetes: A handbook for general practice. Developed by the RACGP and Diabetes Australia, this comprehensive resource features up-to-date evidence and advice to support high-quality care of patients with type 2 diabetes.
