About us
Hunter New England and Central Coast Primary Health Network is a not for profit organisation primarily funded by the Commonwealth government to improve the efficiency and effectiveness of the primary health care system. HNECC PHN supports GPs and other health professionals, and delivers a range of programs. Supporting health care professionals with: •HealthPathways -an online portal of localised clinical pathways •electronic referrals and eHealth records •accreditation and quality standards •chronic disease management and care coordination. We also fund and manage healthcare and support programs for people in the community including: •Closing the Gap Aboriginal Health programs •After Hours GP care •Primary Mental Health Services •Primary Allied Health Services •Health screening and immunisation

The Australian Health, Housing & Homelessness Network is thrilled to see in this year’s MYEFO that funding for the Primary Health Networks (PHNs) Homelessness Access Program has been extended for another year. This funding is vital to improving healthcare accessibility and equity for people experiencing homelessness across the country. The below are some examples of how funding of the Homelessness Access Program has supported vulnerable populations so far: 🔸 PHN Homeless Needs Assessments: These deep dives into region specific homelessness have led to tailored, informed commissioning of services that better meet homeless healthcare needs locally. Wentworth Healthcare Limited Homelessness Needs Assessment is an example of how HAP funding is informing service delivery: https://lnkd.in/gdAu6Aa7 🔸 Primary Health Outreach: Hunter New England and Central Coast PHN commission The Primary Care Outreach Team (PCOT), a program established for those experiencing homelessness due to domestic and family violence, linked to a Specialist Homeless Service (SHS). Partnering with local SHS Coast Shelter, an Outreach Nurse, General Practitioner Speech Pathologist and Maternal Child Health Nurse all of whom have specialist training and skills in working with patients provide trauma-informed health assessments, screening and intervention, case management, access to medication and referrals to specialist services. (Source: HNECC After Hours Activity Summary 22/23-26/27) 🔸 Homelessness Health Framework: A macro-level guide for PHNs to plan and implement effective strategies, improving health and wellbeing outcomes for people experiencing homelessness. With individuals experiencing homelessness having an average life expectancy of just 44 years, the urgency for this work cannot be overstated. The A3HN looks forward to continuing our partnership with the Australian Government Department of Health and Aged Care & Central and Eastern Sydney PHN in efforts to bridge health inequities and deliver better outcomes for vulnerable Australians.
