🚨 Uncovering the Cardiovascular Impact of BRAF/MEK Inhibitors: Why Cardio-Oncology Matters 🚨 The advent of BRAF and MEK inhibitors has been a game-changer in the treatment of metastatic melanoma, and their role is expanding to other cancers, including colorectal cancer. With the growing use of these therapies, however, comes a critical concern: cardiovascular adverse events (CVAEs). A systematic review and meta-analysis of 5 randomised controlled trials (RCTs) involving 2,317 melanoma patients sheds light on the significant cardiovascular risks associated with BRAF/MEK inhibitor therapy compared with BRAF monotherapy. Some key findings include: 🔴 Pulmonary Embolism: 4.36x increased risk 🔴 Decreased Left Ventricular Ejection Fraction (LVEF): 3.72x increased risk 🔴 Arterial Hypertension: 1.49x increased risk 🔴 Younger Patients Under 55: 26.5x higher risk of LVEF reduction As cancer therapies evolve, the cardiovascular side effects that come with them must not be overlooked. Cardio-oncology is now a critical part of comprehensive cancer care. Identifying patients who are at higher risk for cardiotoxicity—such as those with certain pre-existing cardiovascular conditions or prior cardiotoxic treatment, is prudent to encourage safer treatment administration. 🚨 Blood Pressure and Cancer: A Match Made in Mortality?🚨 When we think about cancer, blood pressure often doesn’t come to mind. But it should. High blood pressure (hypertension) is more common in cancer patients than in the general population—and research has demonstrated that it's a real risk factor for both heart disease and cancer-related mortality. 🫀🎗️ Studies have shown that high blood pressure is associated with higher cancer-related mortality rates and may increase the risk of certain types of cancer. Moreover, untreated hypertension can complicate cancer treatment by raising the risk of heart failure, particularly with therapies like anthracyclines, ibrutinib, and VEGF inhibitors. This makes early detection and effective management of blood pressure in cancer patients essential. But here’s the crucial point: Hypertension isn’t just a short-term concern. It can develop during cancer treatment—or even years later. Whether it’s adjusting treatment plans, closely monitoring blood pressure or referring patients to a cardiologist, timely intervention can improve health outcomes. ⏱️👨⚕️ 💡 A large, long-term study from the Metabolic Syndrome and Cancer Project, which included over 577,000 participants across Norway, Austria, and Sweden, found that higher blood pressure levels were associated with increased cancer risk and mortality. Interestingly, the relationship between blood pressure and cancer outcomes was stronger in men compared to women. While routine blood pressure management may seem straightforward, in the context of cancer care, it is anything but. "To treat or not to treat—that is the question!" 🤔 But is heart disease making that decision for you? ⚡ Interesting study alert! ⚡ It has been reported that between 5% and 43% of cancer patients have pre-existing cardiovascular disease (CVD). However, studies on how CVD impacts treatment regimes and survival are limited. A 2020 study explored how pre-existing CVD affected cancer treatment patterns and survival in lung cancer patients. This large Canadian study of 20,689 patients used statistical models to observe the relationship between CVD and cancer treatment decisions. The median age at diagnosis was 70 and women constituted 49% of the cohort. Approximately half of patients had distant metastases at diagnosis. Here’s a few things they found: 🔹 Lower Treatment Rates: Patients with CVD were less likely to receive surgery (11.2% vs. 18.9%), chemotherapy (16.4% vs. 32.3%) or radiotherapy (26.3% vs. 32.1%). 🔹 Shorter Survival: Overall survival (OS) was significantly lower for those with CVD (5.3 months vs. 9.2 months), as was cancer-specific survival (CSS) (6.0 months vs. 9.8 months). 🔹 Higher Non-Cancer Mortality Risk: CVD increased the risk of non-cancer-related death by nearly 50%, underscoring the need for well-rounded care. 💡 Why does this matter? The authors hypothesised that CVD could have affected treatment options and ultimately patient outcomes—though certainly other factors would be at play (read the full study for details and limitations!). While the study focused on lung cancer, these findings could potentially apply to other cancers as well. 👉 Takeaway: By managing cardiovascular and cancer conditions together, early cardio-oncology engagement has the potential to help patients access a wider range of cancer treatments in a safer manner.
