More than 100 member organizations working to achieve health equity through comprehensive primary health care in Ontario
About us
The Alliance for Healthier Communities is the voice of a vibrant network of community-governed primary health care organizations in Ontario. Member organizations serve diverse communities across the province, and are rooted in the communities they serve. We share commitment to advancing health equity through the delivery of comprehensive primary health care. We are an organization and a movement built on the belief that health is “a complete state of physical, mental and social wellbeing and not merely the absence of disease and infirmity.” Through comprehensive primary health care, we aim to eliminate barriers that leave 3.5 million people in Ontario at risk of poor health. Together, we work to address the root causes of illness by changing social, economic and environmental policies to correct inequities harming people’s health.

Workers in community health care across Ontario continue to face significant wage gaps. These wage gaps continue to make it harder to retain essential and dedicated community health staff who support health and wellbeing across Ontario, and difficult to fill vacancies in key roles across vital community health organizations and teams. To ensure we can reach the vision of each and every Ontarian being connected to primary health care, we need government leaders to commit to closing the gaps in wages that community health workers face across the province. Please join us in taking a moment to send a message to the Ontario government to #CloseTheWageGap in support of community health: https://lnkd.in/gwY_KR-b

Today is National Day of Remembrance and Action on Violence Against Women. Across Ontario and Canada, women and girls continue to experience violence and intimidation that have profound impacts on their lives and health, and of their children, families and communities. For racialized women and girls and others who face marginalization, misogynistic mindsets and actions perpetuate oppression and inequities. This must end NOW. We all have a responsibility to take action. Here's a good starting point: https://lnkd.in/gqHhyTBK As we remember women and girls murdered and traumatized by male violence, let's also focus on current and future generations, on the actions we need to take to imagine a world that's free of misogyny, where all women and girls are safe and can thrive. #16DaysOfActivism

On November 26 at Queen's Park, community health leaders from across Ontario shared a message of #equity and #access to increase primary care attachment w/ Ontario MPPs gathered for the Alliance's Community Primary Health Care Day. We showcased proven solutions to increase attachment to #PrimaryHealthCare and reduce overall health system costs. Rexdale Community Health Centre brought their mobile care bus right to the legislature from Doug Ford's home riding, to showcase what nimble, equity-driven approaches to increasing access to primary health care in Ontario can do. With the right support and funding, Ontario can overcome the crisis in #PrimaryCare and create a more sustainable, equitable health care system. #ONpoli #Healthcare #ONhealth

How connected (or not) we feel to the wider community and institutions around us is a key driver of personal, family, community and ultimately national and societal health. We all benefit when we've created environments in which to live, work and play where people feel welcome and have lots of ways to participate and belong. We're proud to present a session that draws on local community level expertise of Alliance members to deliver a FREE webinar: next Weds, November 27 at 12 p.m. ET: BEYOND ELECTIONS: Promoting health and well-being through civic engagement. A panel of community experts will cover addressing barriers to civic and democratic engagement, including planning for community events, increasing volunteer levels, and all the ways we connect outside of the ballot box. For the #mentalhealth needs of people in Ontario, we need Ontario's leaders to close the wage gap, NOW. It's the only way to ensure that organizations can retain and recruit the staff needed to keep services open for Ontarians. #ONpoli #ONhealth
We’re feeling the strain with so many vacancies and so few resources. With 83% of community health organizations reporting longer wait times due to staff shortages, we need action. It’s time to close the wage gap. #ForUsForYou

Love to see Alliance members working w/ physician partners to ensure that increased access to primary health care in Ontario puts the needs of patients and their experiences at the centre of health system transformation. #ONhealth #PrimaryCare
Our Primary Care Neighborhood Network partners had a great day today collaborating about the implementation of the increased access to primary care that we are achieving with Dominik Nowak, MD from the Ontario Medical Association. We talked about the hope and optimism that comes from implementing new models of care and putting patients and the primary care experience at the centre. We also talked about the realism of the challenges in front of us in implementing this kind of transformational change and how we can work together to create sustainable change. WellFort Community Health Services and our partners are trailblazing in our new Population Health Clinics and shared resourcing for increased access to team based care. This is how we are creating a Healthier Community for All. Stay tuned to watch our impact! Tajman G. Anthony DiValentino Rachita Gurtu Jill Shakespeare, MSW, RSW, MBAAnne Marie Lang-Berkowitz

Today is Transgender Day of Remembrance. As we grieve along w/ the loved ones for trans lives lost in the last year, we also stand in steadfast solidarity w/ #Transhealth leaders and advocates. Because ALL health care must be gender-affirming. A key development in Ontario, supported by the work of @OntarioHealthOH and others, is the development of new Quality Standards for Gender-Affirming Care for Gender Diverse people. Read more here: https://lnkd.in/gkwZU-5W That work builds on the work for many years by @RainbowHealthOn and its partners, including the Alliance and many of our members, to educate and advocate for safer care for gender diverse people across our entire health system. Learn more here: https://lnkd.in/ezKqYbh6 In the last 2 years, the Alliance and our members have contributed to resources to make cancer screening more accessible to gender diverse people. Here's more on that work, and a list of other resources to support taking action in your organization: https://lnkd.in/g2EutR2Z #TDOR #TransDayofRemembrance #TransHealth #2SLGBTQHealth #TransRightsAreHumanRights
