What is a Family Health Team? In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients. Who is the Hamilton Family Health Team (HFHT)? HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. We serve over half of the population of Hamilton. There are many opportunities for innovation, leadership, and collaboration with other community health agencies at our public-sector organization.
