Great strides have been made in HIV/AIDS treatment and prevention since the height of the epidemic in the 1980s, with the advent of highly active antiretroviral therapy and the Treatment as Prevention (TaSP) Strategy, pioneered by Dr. Julio Montaner, Executive Director and Physician-in-Chief of the BC Centre for Excellence in HIV/AIDS (BC-CfE) The BC-CfE is Canada’s largest HIV/AIDS research, treatment and education facility, and is nationally and internationally recognized as a world leader in combatting HIV/AIDS and related diseases. It is easy to take for granted the many advances made in HIV treatment and prevention since the advent of highly active antiretroviral therapy. Unfortunately, many people still don’t know that HIV is different now, which can fuel stigma and discrimination and make it harder for people to get tested and access treatment. If we are to meet the first target in ending the AIDS epidemic, where every person knows their HIV status, we must change the narrative. Here are a few ways that our conversation around HIV has changed in the past decade: · Getting tested and knowing your HIV status is no longer something to fear, but rather it helps protect your health and that of your partners. Whether you test positive or negative, knowing your status opens up the conversation around treatment options that keep you and your loved ones healthy. · For those living with HIV, there are simple, effective treatment options that lower your viral load to undetectable levels, keeping you healthy, averting the onset of AIDS-defining illnesses, and preventing you from passing HIV to your partner. In BC, these medications are fully covered by the government at no cost to the individual through the Drug Treatment Program. · This proven strategy is known as Treatment as Prevention or TasP, and forms the scientific basis behind the groundbreaking U=U campaign. U=U or Undetectable=Untransmittable has bridged scientific consensus with community engagement, empowering people living with HIV to take control of their well-being and make informed choices about their sexual health. · For those who are HIV-negative but at-risk of acquiring HIV, Pre-Exposure Prophylaxis (PrEP) involves taking a medication that almost fully prevents HIV. Here in BC, this medication is fully covered for eligible individuals through the BC-CfE’s PrEP Program. · Where an HIV exposure can’t be prevented, Post-Exposure Prophylaxis (PEP) is a medication that can be taken up to 72 hours after exposure and is effective in preventing HIV. Providence Living is pioneering an approach that supports residents to define what living well in long-term care looks like. This non-profit operator has worked closely with the Distinguished Scholar in Seniors Care, Dr. Amy Salmon, to implement an ambitious and innovative social relational model of care. The shift involved reimagining staffing model, job descriptions, organizational culture, and most importantly, resident autonomy. The multi-disciplinary Seniors Research Group at the Centre for Advancing Health Outcomes, led by Dr. Salmon, has through their data collection and dissemination, supported Providence leadership and staff with critical decision making. Providence Living at The Views in Comox, BC is first-of its kind, but the model will continue to be adapted to other long-term care settings under the Providence umbrella.
