Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and diverse communities to achieve wellness. Sherbourne has been serving the diverse communities of southeast Toronto and beyond since 2002. In addition to providing primary care, we offer our clients mental health services in both individual and group settings, outreach support, complementary care (chiropractic, acupuncture, naturopathy), and chronic illness management services. We are widely recognized for our efforts to reduce social, economic and other barriers to health care. For example, our mobile bus program delivering health care directly to people outside our walls and our Acute Respite Care (ARC program for homeless and under-housed people are unique Canadian programs that have brought us international acclaim. As well, our leading-edge health and support services for lesbian, gay, bisexual, trans, 2-Spirit and queer (LGBT2SQ) communities attract attention around the world. At Sherbourne Health, we believe that to truly build healthy individuals and communities, services must extend beyond basic health care. Through key partnerships, we address the social determinants of health that are the root causes of many health problems. Since 2007, we have been serving clients through our Family Health Team that brings together healthcare providers and practitioners from different disciplines to coordinate the best possible care for a particular patient. At Sherbourne, a Family Health Team can comprise physicians, nurses, health promoters, mental health counsellors, outreach workers, dietitians and client resource workers, all tailored to the needs of the client. Sherbourne combines high quality clinical care with responsive, culturally appropriate community development programs that bring low-barrier services to people in the surrounding communities who need it most.

Our Bridging the Gap campaign has been going strong! We've raised over $17,000 for Supporting Our Youth (SOY) so far, and our online auction, which is now closed, was a huge hit. Thanks to the 90 participants who bid on items and over 35 businesses that contributed—we couldn't have done it without you! We also want to thank everyone who donated and helped spread the word. This year, to honour National Day for Truth and Reconciliation, our WINK program partnered with Indigenous tattoo designer Ameena Buchanan for a poignant orange shirt design. Every aspect of this collaboration is a tribute made with care and intentional symbolism. Central to the design is the phrase “Gichi-apiitendaagoziwag gakina abinoojiiyag.” This means "Every Child Matters" in Anishinaabemowin, the Ojibwe language, translated by the artist's mother. These shirts were screenprinted by staff and shared with our WINK attendees during the latest WINK session. This special activity was part of the program’s commitment to better serving its many Indigenous clients, and embedding truth and reconciliation into our services. If you are interested in purchasing a shirt, a limited number are available for sale in STASH, located on our first floor. Funds go to the artist, with the bulk of proceeds going towards Council Fire's Youth and Youth Wellness program. Our biggest thanks to Ameena Buchanan for working with us on this project and her support of WINK. Keep an eye out later this week for more details on this project and the artist, as well as other initiatives at Sherbourne Health related to the National Day of Truth and Reconciliation.
