This commentary published today in The Lancet Global Health examines an initiative that is exploring new ways to identify grand challenges – and how to engage those most affected by them. At its core is a global survey developed through a participatory dialogue between Grand Challenges Canada and the Teach to Reach community, network, and platform of more than 24,000 health professionals. Most Teach to Reach participants work in districts and local communities of the countries hardest hit by climate change. They have been sharing observations and experience about climate impacts on health in their communities since July 2023. The survey’s development process offers methodological insights into how participatory approaches can strengthen the identification of critical barriers in complex global challenges. The pilot phase demonstrated how participatory processes can strengthen instrument design. 306 health workers across multiple countries tested the survey in December 2024, providing feedback that led to substantive revisions. Perhaps most significantly, the process introduced new questions about epistemology - asking respondents to clarify the basis for their knowledge claims, whether from personal experience, professional observation, or research. This methodological innovation emerged directly from dialogue with health workers about how they know what they know about climate impacts on health. The development process also catalyzed new forms of local collaboration. In a remote area of Democratic Republic of Congo, for instance, a group of civil society organization leaders gathered to review the survey together, generating collective insights that helped ensure the instrument would resonate across diverse health system contexts while maintaining methodological soundness. The methodology demonstrates three key aspects of participatory survey development: First, it shows how structured peer input (open and at scale, not pre-selected or limited to small focus group discussions) during development can strengthen instrument design by surfacing nuanced understanding of how challenges manifest in different contexts. Second, it provides a framework for bridging systematic data collection with experiential knowledge, potentially informing more responsive approaches to identifying critical barriers. Third, the process suggests new possibilities for strengthening the grand challenges model itself - demonstrating how systematic barrier identification can itself become a process that builds engagement with the communities striving to overcomes these barriers. Co-créer avec des communautés locales pour pérenniser les interventions de santé communautaire ✨ La Cellule d'animation communautaire (CAC) est une structure multisectorielle de coordination des initiatives communautaires sous le leadership du chef du village. Elle joue un rôle clé dans : 1. la promotion auprès des familles et des communautés, l'adoption de pratiques familiales essentielles pour la survie, la protection et le développement de l'enfant ; 2. la promotion de la demande et l'utilisation des services de base et incite le changement social favorable ; 3. l'habilitation des communautés pour assurer leur capacité d'agir pour leur développement. Cependant, maintenir des CAC actives, engagées et dotées des compétences appropriées est un véritable défi compte tenu du nombre des CAC et de nombre des interventions nécessaires. Garder les membres des CAC motivés demande du soutien. Afin de rendre dynamique les CAC de la Zone de Santé de Kisenso, nous avons appuyé ce 20 janvier 2025, la mise en place de 4 CAC modèles dans l'Aire de Santé (AS) de la Paix et formé plus de 30 membres de ces dernières sur : 📌 Les paquets minimum des attributions d'une CAC et d'un relais communautaire ; 📌 Le diagnostic communautaire et l'élaboration d'un plan d'action communautaire ; 📌 La stratégie OVM (Occasions de Vaccination Manquées). A l'issue de cette activité, utilisant la technique de triple A (Analyse des problèmes, analyse des causes, agir/actions), chaque CAC a élaboré son plan d'action communautaire et des solutions locales ont été adoptées par les participants pour renforcer la vaccination de routine, la surveillance à base communautaire et les activités communautaires dans l'aire de santé.
