Para describir los resultados de un modelo de atención transicional remota conducido por personal de enfermería para mejorar el acceso a la atención luego de la UCI, Naiditch H et al analizaron los datos de más de 32 000 pacientes de alto riesgo (mediana de edad 74a, 67.6% blancos, 95.8% varones) que recibieron esta atención luego de la UCI a través de la mortalidad al año del egreso y los días libres de hospitalización hasta el día 90. Encontraron que estos pacientes tuvieron 2d menos en casa y eran más propensos a morir dentro de los 90d posteriores, mientras que los que recibieron atención transicional por el personal de enfermería para mejorar el acceso a la atención luego de la UCI tuvieron una mediana de 4d o más en casa dentro de los 90d posteriores y eran menos propensos a morir dentro de estos 90d (RRA 16.1). Concluyeron que la atención transicional conducida por personal de enfermería es viable y tiene el potencial de mejorar el resultados de los pacientes que egresan de UCI #telehealth #nurseled #ICU

Big changes are on the horizon! Starting October 1, Ohio is set to adopt the Patient-Driven Payment Model (PDPM), with an enhanced focus on respiratory care in skilled nursing facilities. This new model emphasizes patient conditions and care needs, underscoring the importance of precise documentation and superior care delivery. La adultez mayor tiene que dejar de considerarse como una etapa de enfermedades crónicas y dependencia. Es cierto que esto requiere mayor inversión de recursos en una atención de calidad y profesionales de salud mejor capacitados, pero es lo que corresponde, una atención centrada en la persona y su bienestar.
