Who do you know? Heart failure is often an invisible disease, impacting millions worldwide while frequently coexisting with other conditions like diabetes, kidney disease, obesity, and mental health challenges. These connected conditions (comorbidities) complicate health management, and highlight the importance of holistic, patient-centered care. Global Heart Hub is dedicated to bringing visibility to these challenges and supporting those living with heart failure and its associated conditions. Together, we can make the invisible visible. #InvisibleDisease #Comorbidities #ConnectedConditions #HeartHealth Watch our Who Do You Know video and learn more about heart failure - https://lnkd.in/ehqzhDHd

It was an honour to represent Global Heart Hub today at the meeting "Rising to the challenge: Time for action on cardiovascular health" organised by the European Alliance for Cardiovascular Health (EACH) in Brussels. We heard the experiences and perspectives from patients, families, clinicians and policymakers in a very powerful multi-stakeholder meeting. It became clear that not only is a holistic Cardiovascular Health Plan focusing on primary and secondary prevention, early detection and diagnosis, early treatment and rehabilitation necessary, but that it is the most cost-effective way to tackle the leading cause of death in Europe. The Cardiovascular Health Plan is already here and there is no better time to act than now!

A fantastic close by Victoria Tzouma after an inspiring afternoon. Today we called on the European Union to create a holistic #cardiovascular health plan that focuses on both primary and secondary prevention, early detection and diagnosis, as well as early treatment and rehabilitation. There are best practices from national health plans that we can learn from, and today our panelists gave us some important tips: - we need political commitment - cross-sectorial collaboration - concreate financial plans It is time for action. The time is now. As the Rijken family said today "let's make the change now". Thank you European Alliance for Cardiovascular Health (EACH) #CVHealthForAll
