In the final series of our “𝐌𝐞𝐞𝐭 𝐭𝐡𝐞 𝐒𝐩𝐞𝐚𝐤𝐞𝐫𝐬” from ICMS 2024, Serosep were delighted to host Dr Annie Joseph. Annie is a Consultant Microbiologist and Laboratory Lead at Nottingham University Hospitals, with a particular interest in urological infections and women’s health, particularly on complicated and recurrent UTIs.    🧫 𝐔𝐑𝐈𝐍𝐄 𝐓𝐑𝐎𝐔𝐁𝐋𝐄: 𝐔𝐓𝐈 𝐃𝐈𝐀𝐆𝐍𝐎𝐒𝐓𝐈𝐂𝐒 𝐈𝐍 𝐓𝐇𝐄 𝟐𝟏𝐒𝐓 𝐂𝐄𝐍𝐓𝐔𝐑𝐘   👉 𝐁𝐞𝐲𝐨𝐧𝐝 𝐓𝐫𝐚𝐝𝐢𝐭𝐢𝐨𝐧𝐚𝐥 𝐓𝐡𝐫𝐞𝐬𝐡𝐨𝐥𝐝𝐬: Historically, the “Kass threshold” of 10^5 CFU/mL has been used to identify significant bacteriuria. However, Dr. Joseph challenged this norm, emphasizing that even low bacterial counts could indicate UTIs, particularly in women with recurring symptoms.   👉 𝐓𝐡𝐞 𝐔𝐫𝐢𝐧𝐚𝐫𝐲 𝐌𝐢𝐜𝐫𝐨𝐛𝐢𝐨𝐦𝐞: Recent research suggests that urine may not be sterile, with the urinary microbiome playing a role in urinary health. Dr. Joseph highlighted the need to move beyond the outdated “sterile urine” model and consider microbial diversity in diagnostics.   👉 𝐋𝐢𝐦𝐢𝐭𝐚𝐭𝐢𝐨𝐧𝐬 𝐨𝐟 𝐂𝐮𝐫𝐫𝐞𝐧𝐭 𝐓𝐞𝐬𝐭𝐬: Standard culture methods are often insufficient for accurately diagnosing UTIs, especially in vulnerable populations. Dr. Joseph advocated for methods that go beyond traditional microscopy, culture, and sensitivity (MC&S), including emerging genomic techniques.   👉 𝐅𝐨𝐜𝐮𝐬 𝐨𝐧 𝐔𝐧𝐝𝐞𝐫-𝐒𝐞𝐫𝐯𝐞𝐝 𝐆𝐫𝐨𝐮𝐩𝐬: Vulnerable patients, such as the elderly, those with cognitive impairments, and individuals with indwelling prosthetics, are often poorly served by current diagnostics. Addressing their needs could greatly improve outcomes in managing UTIs.   👉 𝐀 𝐏𝐚𝐭𝐡 𝐅𝐨𝐫𝐰𝐚𝐫𝐝: Embracing modern techniques, including proteomics and advanced culture methods, could revolutionize UTI diagnostics. In our second last episode of our “𝐌𝐞𝐞𝐭 𝐭𝐡𝐞 𝐒𝐩𝐞𝐚𝐤𝐞𝐫𝐬” Series from ICMS 2024, we would like to introduce Dr Kirsty Dodgson, a Consultant Clinical Microbiologist with over 20 years at Manchester University NHS Foundation Trust. Kirsty has a strong focus on Outpatient Antimicrobial Therapy (OPAT) and Critical Care, and her presentation delved into Manchester’s CPE journey and the strategies they used for managing this growing threat.    🦠 𝐂𝐏𝐄: 𝐓𝐇𝐄 𝐌𝐀𝐍𝐂𝐇𝐄𝐒𝐓𝐄𝐑 𝐄𝐗𝐏𝐄𝐑𝐈𝐄𝐍𝐂𝐄   👉 𝐂𝐏𝐄 𝐎𝐮𝐭𝐛𝐫𝐞𝐚𝐤 𝐢𝐧 𝐌𝐚𝐧𝐜𝐡𝐞𝐬𝐭𝐞𝐫: Since the early 2000s, Manchester has faced rising cases of CPE, driven by limited national guidelines and a lack of established screening protocols. By 2014, Manchester hospitals were processing 6,000 CPE screens per month, highlighting the scale of the issue and the need for robust management protocols.   👉 𝐄𝐯𝐨𝐥𝐮𝐭𝐢𝐨𝐧 𝐨𝐟 𝐒𝐜𝐫𝐞𝐞𝐧𝐢𝐧𝐠 𝐚𝐧𝐝 𝐓𝐞𝐬𝐭𝐢𝐧𝐠: Initially, screening relied on basic enrichment methods, but as cases increased, Manchester introduced PCR-based tests to improve accuracy and turnaround times. By 2016, they transitioned to using the Cepheid PCR for all screening, increasing lab efficiency and improving patient flow through rapid results.   👉 𝐂𝐡𝐚𝐥𝐥𝐞𝐧𝐠𝐞𝐬 𝐢𝐧 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐒𝐜𝐫𝐞𝐞𝐧𝐢𝐧𝐠: Screening criteria required adaptations due to Manchester’s unique epidemiology. For instance, patients admitted within the past 12 months to any healthcare facility were screened upon arrival, covering nearly 40,000 patients annually.   👉 𝐅𝐢𝐧𝐚𝐧𝐜𝐢𝐚𝐥 𝐚𝐧𝐝 𝐂𝐥𝐢𝐧𝐢𝐜𝐚𝐥 𝐈𝐦𝐩𝐚𝐜𝐭: The financial burden of managing CPE is substantial. A study of 60 CPE bacteraemia cases revealed an average cost of £14,694 per patient. Infected patients also experience longer hospital stays, further adding to healthcare costs and patient management complexity.   👉 𝐑𝐞𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐌𝐞𝐜𝐡𝐚𝐧𝐢𝐬𝐦-𝐁𝐚𝐬𝐞𝐝 𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭𝐬: With few treatment options for CPE, Manchester has developed resistance mechanism-based treatment strategies. New antibiotic combinations like Ceftazidime-Avibactam and Meropenem-Vaborbactam, tailored to specific CPE genotypes, are showing promise for effective management.   📢 Kirsty’s presentation highlighted the crucial role of adaptable screening protocols, rapid diagnostics, and targeted treatments to manage CPE effectively. Next in our "Meet the Speakers" series from Serosep’s ICMS 2024 Symposium is Kelly Ward, Interim Head of Operations for Public Health Wales Infection Services. Kelly shared insights on creating sustainable microbiology labs, drawing from 20+ years of NHS experience, with 15 in leadership. ♻️  𝐌𝐢𝐜𝐫𝐨𝐛𝐢𝐨𝐥𝐨𝐠𝐲 𝐢𝐧 𝐏𝐮𝐛𝐥𝐢𝐜 𝐇𝐞𝐚𝐥𝐭𝐡 𝐖𝐚𝐥𝐞𝐬: 𝐇𝐨𝐰 𝐚𝐫𝐞 𝐰𝐞 𝐭𝐫𝐲𝐢𝐧𝐠 𝐭𝐨 𝐚𝐜𝐡𝐢𝐞𝐯𝐞 𝐥𝐚𝐛𝐨𝐫𝐚𝐭𝐨𝐫𝐲 𝐬𝐮𝐬𝐭𝐚𝐢𝐧𝐚𝐛𝐢𝐥𝐢𝐭𝐲? 👉 𝐓𝐡𝐞 𝐏𝐇𝐖 𝐃𝐞𝐜𝐚𝐫𝐛𝐨𝐧𝐢𝐬𝐚𝐭𝐢𝐨𝐧 & 𝐒𝐮𝐬𝐭𝐚𝐢𝐧𝐚𝐛𝐢𝐥𝐢𝐭𝐲 𝐏𝐥𝐚𝐧: PHW aims for net-zero emissions by 2030 and carbon-negative by 2035, focusing on carbon management, estates, transport, procurement, and service adjustments, aligning with the Well-being of Future Generations (Wales) Act 2015. 👉 𝐑𝐞𝐝𝐮𝐜𝐢𝐧𝐠 𝐏𝐥𝐚𝐬𝐭𝐢𝐜 𝐚𝐧𝐝 𝐖𝐚𝐬𝐭𝐞: One key goal is for microbiology to be plastic-free by 2030. Initiatives include trials of reusable PPE, reducing single-use plastics, and conducting life-cycle assessments (LCA) on lab consumables. These actions help identify waste reduction opportunities, like recycling pipette tips and switching to biodegradable materials, that can scale across health boards. 👉 𝐂𝐡𝐚𝐥𝐥𝐞𝐧𝐠𝐞𝐬 𝐚𝐧𝐝 𝐏𝐫𝐚𝐜𝐭𝐢𝐜𝐚𝐥𝐢𝐭𝐢𝐞𝐬: Transitioning to sustainable practices comes with challenges, especially when testing new equipment like the Bactizapper sterilisation system. Although it offers environmental benefits, it has introduced safety and compatibility concerns. Kelly stressed the importance of trial periods and evaluating new processes to maintain service continuity. 👉 𝐂𝐨𝐥𝐥𝐚𝐛𝐨𝐫𝐚𝐭𝐢𝐨𝐧 𝐟𝐨𝐫 𝐒𝐮𝐬𝐭𝐚𝐢𝐧𝐚𝐛𝐥𝐞 𝐒𝐨𝐥𝐮𝐭𝐢𝐨𝐧𝐬: PHW’s partnership with Revolution Zero has involved direct engagement with lab staff, management and suppliers to identify waste-reduction strategies. This has led to practical initiatives, like reducing plastic bag usage by switching to double-pocketed bags in collaboration with Cardiff Sexual Health Clinic, resulting in a 90% reduction in plastic waste. 👉 𝐑𝐞𝐜𝐨𝐦𝐦𝐞𝐧𝐝𝐚𝐭𝐢𝐨𝐧𝐬 𝐟𝐨𝐫 𝐭𝐡𝐞 𝐅𝐮𝐭𝐮𝐫𝐞: Kelly concluded with recommendations for PHW to focus on high-impact areas like single-use plastics, paper usage and sustainable procurement practices.
