AbstractAbstract
[en] Full text: The use of radioiodine (131I) in the follow-up of well-differentiated thyroid carcinoma patients for both treatment and diagnosis of recurrent and persistent disease is well known. The combination of two methods, serum Thyroglobulin (Tg) measurement and total body scanning (TBS) using 131I is more useful in finding out residual / recurrent disease. Total thyroid tissue ablation is the desire of any physician after the treatment with a large dose of 131I, but serum Tg level above 10 ng/ml indicates the presence of recurrent disease and a new dose of 131I should be given. We compared these 2 markers in the follow up of 60 patients of well-differentiated thyroid carcinoma in Paraguay from 1995-2000. All the patients underwent total or near total thyroidectomy followed by 131I therapy given between 2 and 6 weeks after surgery. Forty-eight (80%) were without metastases but due to persistent uptake in the thyroidal bed, were treated with ablative dose of 100 mCi of 131Iodine. WBS was performed 5-7 days later. Twelve (20%) with distance metastases were treated with 150 mCi of 131I. Twenty (33%) had persistent uptake in thyroid bed one year after ablation and were retreated with 100 mCi of 131I. All patients were hospitalized and separated for 48 hours at the time of dose administration. Thyroglobulin levels were estimated before and after suspension of replacement therapy with L-Thyroxin for 45 days on the day of whole body scan with 131I and repeated every six monthly. All patients were following up for 5 years. Sensitivity and Specificity of WBS for detecting residual / recurrent disease was 85% and 83% compared to 90% and 84% for thyroglobulin respectively. Taking both the parameters into account, the combined sensitivity was 96% and specificity 98%. Thyroglobulin estimation in patients under treatment with L-thyroxin did not rule out the disease. We conclude that both markers can be used together in the follow up well differentiated thyroid carcinoma and to avoid false negative results, these should be done at the same time after withdrawing L thyroxin replacement. Characterization of a soil x-rays fluorescence method and its relations with the plant-animal system
AbstractAbstract
[en] The characterization of the soils pertaining to three parcels of the enterprise La Bayamesa was carried out using the x-ray fluorescence method in order to detect the deficit of minerals in this region. The total of determined elements was 32. Likewise, the relation of phosphorus, calcium, magnesium, sodium, potassium, copper, zinc and iron content with their levels in the analyzed pastures was established. The results frequently showed deficit level of copper, magnesium, phosphorus, and to a lesser extent of iron in the blood serum. Subnormal values mainly for the phosphorus, were observed in the pastures, while the calcium, magnesium, sodium, and potassium values were inferior to the reference values selected for these soils. Upon completion of these studies in rest of the cattle enterprises, they will serve as the basis to improve the specific mineral formula of the province, and also to make the maps of deficit elements in this region

[en] It is presented the FUESMEN experience in routine monitoring of thyroid internal doses due to inhalation of 131I in workers of the Nuclear Medicine Service in normal operation or accidental exposure. It is used a surface contamination monitor, type Geiger Mueller, calibrated with a acrylic phantom based on specifications of the simulator of thyroid of ICRU 48 with 131I reference activity. Through the obtained measurements is achieved to validate the use of Portable Monitor to carry out preliminary exploration on the monitoring scenarios of incidental situations

[en] tecognition of the Competition of the regulatory authority for the test: Determination of the Committed Effective Dose due to the incorporation of 131I. Product of the low perception of risk of the License Holder, the lack of training of those responsible for security and the lack of standardization and quality assurance in the part of the dose allocation process carried out in the Nuclear Medicine Departments, the individual radiological surveillance of the 131I incorporation in Cuba is not done properly according to the requirements of the Center National Nuclear Safety and international recommendations. Fruit of the consensus between Radiation Protection and Hygiene Center and the National Nuclear Safety Center are They developed recommendations for the Individual Radiological Surveillance of Internal Exposure, which they contain the actions aimed at bridging the identified deficiencies. An approach of system with national scope and stable over time that allows the recognition of the Competence of the regulatory authority even if the dose allocation process is shared between the Department of Nuclear Medicine and the Laboratory of Internal Dosimetry.
