AbstractAbstract
[en] By the end of 1996, the tests of the radiochemical purity (RCP) of preparation of HMDP-99mTc at Necker hospital were effected by means of 'Tech-Kit' technique which allowed making evident only one radiochemical impurity, the free TcO4-. So, the aim of this work has been the search for another technique able to detect also the reduced hydrolyzed 99mTc, and then, to optimize, validate and standardize this new technique. Several methods have been tested and compared on different points (the chemical species identified, practical difficulties, duration, reproducibility) in order to chose 'the best' one, consisting in associating two TLC (migration time, 3 minutes): - TLC no.1, ITLC-SG/Methyl-ethyl-ketone; - TLC no.2, ITLC-SG/1M Sodium acetate. This method has been subsequently: - optimized, concerning the quantity of deposed sampling, way of deposit drying, migration range, reading protocol by radio-chromatograph; - validated, by verifying the migration of radiochemical impurities by means of controls and by evaluating its sensitivity; - standardized, by defining the interest zones for interpreting the radio-chromatograms, by evaluating the reproducibility and by calculating the 'normal values' of RCP percentages. This method has been adopted as routine and with current recoil (40 preparations) and our conditions of realizations, the extreme values of the percentages obtained for the impurity zones of interests, have been the following: - TcO4- zone, [0.39%-1.07%]; - hydrolyzed reduced Tc zone, [0.96%-5.09%]. By taking into account their distribution we calculated a minimum RCP value of 94% to accept the preparation (the RCP being defined as equal to 100% - % TcO4- % hydrolyzed reduced Tc)

AbstractAbstract
[en] Owing to the great technological progress of the last few years, combined with the possibility of using short-lived radioisotopes, nuclear medicine has made new strides in cardiology from both the morphological and kinetic viewpoints. Kinetic studies are described first: cardiac output measurements, radiocardiography, coronary output measurements. This is followed by a study of morphological tests: static using scintigraphic methods, then dynamic with stationary detectors. It is shown that the addition of a data processing unit to the stationary detectors allows the development of analytical methods similar to conventional kinetic tests and opens up vast possibilities for the future. For the analysis of these methods the techniques considered are as far as possible based on the external detection of radiation emitted by a radioactive tracer after simple intraveinous injection, without catheterization. In this field the use of radioisotopes in cardiology offers the means of performing simple investigations which, being harmless for the patient, incur no loss of blood and can be repeated easily and often

Les importants progres realises ces dernieres annees dans le domaine technologique, associes a la possibilite d'utiliser des radioisotopes de courte periode physique ont permis de donner un nouvel essor a la medecine nucleaire en cardiologie tant sur le plan cinetique que morphologique. On a vu tout d'abord les epreuves cinetiques: mesure du debit cardiaque, radiocardiographie, mesure du debit coronaire. On a etudie ensuite les epreuves morphologiques: statiques, utilisant les methodes scintigraphiques puis dynamiques grace aux detecteurs stationnaires. On a vu que l'adjonction d'une unite de traitement d'information aux detecteurs stationnaires permet la mise au point de methodes d'analyse analogues aux epreuves cinetiques classiques et ouvre le champ a de vastes possibilites d'avenir. On s'est limite au maximum pour l'analyse de ces methodes aux techniques basees essentiellement sur la detection externe du rayonnement emis par un indicateur radioactif apres simple injection intraveineuse, sans catheterisme. [en] The gamma-angio-cardiography at equilibrium is the reference method for the calculation of the left ventricular function (FEVG). We have studied the percentage of centro-cavitary activity (PCCA) by myocardium tomo-scintigraphy. The PCCA was calculated on the basis of the measurement of activity at the center of left ventricular cavity (C) of a small-axis medial cross section, divided to myocardium maximal activity (Mmax): PCCA (C/Mmax) x 100. The obtained values were compared with that of isotopic FEVG in 251 consecutive patients who benefited both by a Tl-201 tomo-scintigraphy (stress/redistribution) and a gamma-angio-cardiography at equilibrium. The correlation between PCCA and FEVG was: r = 0.82 (y = 0.75 x + 1.69; P 0.0001). The PCCA is easily obtainable on the basis of Tl - 201 myocardial tomo-scintigraphies and allows a rapid evaluation of the global left ventricular function
