    🏡 We’re searching for a quiet and welcoming 2-bedroom bungalow to help a person with multiple needs find a safe and stable home.   The ideal home would be:   -A single-story bungalow away from loud or busy areas. -In a calm cul-de-sac, close to parks, places to walk, or community spaces. -Ideally in Hertfordshire - Hatfield, Watford, Hemel Hempstead, St Albans, Welwyn, or Hertford - but we’re open to neighbouring towns like Luton, Dunstable, or Amersham.   This is an urgent need, and finding the right home will make an incredible difference to someone ready to take the next step in their journey.   If you’re a landlord who can help or know of a suitable property, we’d be so grateful to hear from you.

    Did you know that nearly 1 in 5 people in the UK face significant challenges when transitioning from hospital to home, often feeling unsupported and isolated? This vulnerable moment deserves more than just medical attention—it requires compassionate, person-centred care to help people reconnect with their families and communities in ways that feel safe and comfortable. Our Community Transition Service ensures that no one has to navigate this journey alone. Ashleigh Fox, our Transforming Care Director, highlights the profound importance of involving families in this process. By working closely with multidisciplinary teams (MDTs) and families, we tailor each journey home to meet the unique needs of the individual, ensuring it’s as caring and supportive as possible. We also offer free discharge assessments to those who may need additional support, helping to bridge the gap in care for those who need it most. In this heartfelt video, we take a look into her daily life, where, with the right support, she is able to navigate her experiences, embrace her independence, and make her own choices. Ashanti’s journey reminds us of the importance of person-centred care and the positive impact of encouraging people to live in a way that feels right for them. Leaf Complex Care

    Meet Ashanti, a 16-year-old girl with lived experience of autism and learning difficulties.    While growing up, Ashanti was becoming easily anxious in crowded spaces or when there were many people around her.   "I met Bea and Yvan while I was on respite care, but back then, I wouldn't feel comfortable going to places like the park, restaurants, etc."   But now, Ashanti visits the places she loves, goes to school, for a walk, swimming, cooks her favourite dishes and plays on her iPad. She also went on a trip to Dubai with her mother and brother.   "I did not like the weather, but I really enjoyed the trip. Together with my brother, Bea and Yvan we went on the safari and I loved it!" This video gives you a glimpse into her everyday life, showing that, despite any challenge, with the right support, you can still make your own choices and live life the way you choose. https://lnkd.in/dTj_3tTj

    Today, our CCG community came together in our comfiest PJs, Pudsey onesies, and cosy outfits to support Children in Need - a fantastic cause that helps make a real difference in the lives of children and young people across the UK. From delicious bake sale treats to generous donations, every little act of kindness helped make a big difference for children and young people across the UK. It’s moments like these that remind us of the power of community and compassion. Thank you to everyone who dressed up, baked, donated, and made today special.
