I was pleased to assist my Resilium colleague Paul McCulloch with the BowTie Game at the Wolters Kluwer workshop at IChemE Hazards 34 conference yesterday. This interactive board game is based on a familiar tank overfill scenario and focusses on the commercial value of Implementing and Sustaining suitable & sufficient Critical Controls to Prevent the occurrence, and Mitigate the effects of the Loss of Containment of flammable liquid in a tank. These events are far too regular and both the Buncefield (Annex 7 of Vol 2) and CAPECO (Appendix B) investigation reports provide lists of related tank overfill incidents that have familiar sequence of events. Our Process Incidents Map [https://lnkd.in/dDjKCVyt] has been updated to show these incidents which use colour coded icons to illustrate the type of effects.
