On Thursday we ran another Human Factors workshop for the super heroes of South Tyneside and Sunderland NHS Foundation Trust. In the workshop we used a systems thinking approach to explore some common problems we see in healthcare and other industries too. Using the Systems Engineering Initiative for Patient Safety (SEIPS), we examined the influencers and impacts of staff not taking sufficient breaks, and issues around the procurement of safety critical equipment. Don’t Lose Your Audience in Investigation Reports We need the ‘people with the power’, who can influence positive change, to read our reports. If we lose them, we’ve lost our argument for improvement. We need to think of our target audience and consider, ‘what do they need?’. We need to think of potential differences in technical knowledge and literacy levels if we’re writing a combined report for families and CEOs. Forward thinking organisations, like Maternity and Newborn Safety Investigations (MNSI) are reviewing and refreshing their report style regularly to address this balance for their audiences. Fabulous folk such as Zoe Munson and Lou Pye QPM have been pivotal in this. Some useful questions to consider to improve your writing for your audiences: - Is the language right and relevant for the readers? - Can you ask yourself ‘WITI’? Why Is This In? —If you don’t know, be brutal to your words, but be kind to your readers and take it out. On Thursday, Systemic Factors Ltd were invited to deliver a session to managers and executives at Serco during their annual Zero Harm Week. Together we explored ‘Enhancing Leadership through Systems Thinking’. Yesterday, we enjoyed running another online session, on a similar theme, ‘Systems Thinking in Investigations’ to HSE managers and investigators. Huge thanks to the teams for their brilliant contributions and to Fiona Liddell IRMCert, CCEP-I for the invite. Here’s what some of the mighty Serco crew said about the sessions: "This was exceptional and highly valuable. It enhances our approach to safety investigation practices while greatly strengthening our investigative capabilities. We recently enjoyed delivering another Patient Safety Incident Investigation course to Derbyshire Healthcare NHS Foundation Trust, they are always engaged and eager to contribute.
