How can HIEs improve population health management?
Population health management (PHM) is a strategy to improve the health outcomes and reduce the costs of a defined group of people, such as patients with chronic conditions, low-income communities, or elderly populations. PHM requires data-driven interventions that address the social, behavioral, and clinical factors that affect health. However, collecting, sharing, and analyzing data across different healthcare providers and settings can be challenging, especially in a fragmented and siloed system. This is where health information exchanges (HIEs) can play a crucial role.
