Few key things from my perspective a) have we reviewed all test cases against user scenarios - are all scenarios adequately covered b) do we have all the positive as well as negative scenarios c) have we covered the requirements traceability matrix and are all requirements covered in test cases

To identify gaps: - Perform exploratory testing to find bugs, if bugs are found verify it against the test cases related to feature. If a test case is not present related to that bug, then add that particular test case.
