Be sure you're using the right control chart - did you use your control chart flow chart? what kind of data do you have? Variable/continuous, or attribute/discrete? Interpretation of a control chart also needs to be done carefully. Also, if you use breaks in the control chart to delineate the before and after effect of a change, be careful drawing conclusions unless you're certain no other exogenous factors are at play. For example, we made improvements in checking account enrollment approval rate, which showed up as an out of control improvement. We put a break in the chart to delineate the improvement, but we also made sure nothing else drove the change (e.g. changes in marketing or fulfillment processes).

For processes with complex variables or multiple sources of variation, X-bar and R charts may not be sufficient.
