As I know where and what type of errors are occurring. The first time I explained the error with the solution to the team. If again error occurs the same then will ask the team to find out the error by themself. If they are unable then guide them how to address the errors and make strategies to prevent errors from recurring. Motivate the team by explaining what's impact on projects due to these types of errors.

Bring them together for a "toolbox training" session to get them aligned to the project objectives and milestones. It is also important that each team member is fully aware of his role and responsibilities and how it interfaces with the rest of the team. Good and timely communication is paramount.

The key is having the right team, one bad apple can bring down an entire team. Second communications is critical. Third put the energy up front in the planning/scope development phase followed by key stakeholders buy in.

Developing a thorough risk management plan is indeed a proactive measure that can save countless hours and resources down the line. It's also crucial to treat risk management as a living process, constantly evolving with the project's progression.

Risk management is a continuous process. Risks do not remain static. One mechanism to build rigour around the identification and tracking of risks is to build them into a RAID log - this can sometimes help to improve discipline about review and management. Alternatively, if the risk requires more attention, building an FMEA with not only current likelihood and impact, but also include the potential likelihood and impact once all of the actions are taken can you to measure progress towards a destination. The current vs potential gives a clear goal for the team to achieve and helps to articulate the benefit of completing those actions.

Proper training beforehand can avoid majority of mistakes. It brings the whole team on the same page, and enhances communication among members. A training session that invites people to come forward and ask questions, boosts their morale and improves the overall culture. Another thing that I found really helpful is to create a checklist of the common mistakes and make it available to the team. This empowers them to check their own work before then send it forward. And lastly, if there are still mistakes, address them with leniency a few times. This boosts the well being of an employee and naturally make him/her more engaged in work.

Always be positive; sound positive and give compliments; look to the skills of each team member (even if they are better that you are at something); rotate the team through leading roles on a scheduled basis; encourage team members to coach each other; foster a culture to positive honest feedback welcome in sprint retros; laugh with your team; be their servant master. They are here to do the work, you are here to clear the obstacles to that. Give people respect.

1. First, I would not generalise the common mistakes and mitigation techniques; Each project is unique and dynamic. 2. Is it my old team or a new team? Has the team dynamic changed? Is it because of the project, organisation or something in the personal space? 3. Address the common mistakes with possible solutions. Just communicating and discussing the mistakes won't solve the problem. You have to come up with possible solutions together and let your team member come up with a solution they think is best for them. It can be in a one on one setting or during a team meeting/call (depending on the scenario and type of mistake). 4. Never let out in the open, especially to a stakeholder about the problem your team is facing. It is your team!

When I see my project team making the same mistakes, I call a quick meeting. It's like a friendly chat where we talk about what's going wrong. We all share ideas on how to fix it and make sure it doesn’t happen again. I make sure everyone feels okay to speak up. Sometimes, I even ask someone from a different team to give us new ideas. It's all about figuring it out together and getting better.
