The issues I see is that many insights at the end of a project are simply left said with no action in future projects. It is important to collect feedback, offer feedback and do the necessary changes inside the organisation to prevent future issues. Tech and non-tech feedback is equally important, so taking actions should affect both areas. Another area that suffers is the lack of communication between tech and non-tech team members which in most teams has to be mediated by a project lead, but in a perfect world such issue would simply be avoided by following these 5 steps.
