From my experience, it's invaluable to have a clearly defined Definition of Done (DoD). Ensure that the DoD for each backlog item is clear. If it's not clear what "done" looks like, there's a risk the item won't meet the product owner's expectations.

The development team can discuss the identified risks in the daily standup. These risks becomes part of the product backlog and shall be prioritised. Risks, and by implication the work required to mitigate each risk, are managed as work items in work item backlog/list/pool. Use a risk burndown chart to track the progress of risk mitigation. I believe there's no point in having a list of risks if steps aren't taken to plan and address them. This proactive approach ensures the timely and strategic resolution of potential challenges. Everybody will be glad they managed to hear about it in time, and the stakeholders and sponsors well be happy to know it is already mitigated.

Regular sessions with the client & stake holders / business proposal team / development team will keep everyone to be aligned towards a common goal and maintain the transparency. Everyone in the team should aware of scope creep and cross skilling the required knowledge will help us to reduce the risk. Even though scrum places a strong emphasis on working software over comprehensive documentation, sometimes lack of documentations also lead to risk. So minimal documentations on risk factors/ technical debt / pain areas should be necessary.

Utilizing empirical data and metrics from tools like JIRA or Confluence, I recommend conducting a dedicated portion of the sprint retrospective to focus solely on risk assessment. This structured review ensures that not only are new risks identified and existing ones updated, but that the team also reflects on risks that were successfully mitigated or averted. By doing this, the team can create a repository of “lessons learned,” enabling better foresight in future sprints and leading to a more proactive, rather than reactive, approach to risk management.

Within Scaled Agile frameworks, managing risks in your backlog is crucial for project success. Implement risk identification within your regular backlog refinement sessions. Prioritize risks based on potential impact and probability, integrating them into your sprint planning. This is a topic that is very dear to my heart. The essence of Agile is Transparency - Inspection - Adaptation. We make risks transparent by identifying, capturing, categorizing and radiating information about them by putting them on dashboards and other artifacts. Once we have done, this, we inspect them in Risk Review calls, Retrospectives, Team meetings, impromptu conversations or even individually. At this point, we should not solely focus on addressing the risks, but we should look to see what these risks are telling us about our product, our process, and our organization. These insights would help identify patterns which can drive bigger improvement efforts rather than fixing individual risks.

This whole article is not about risk management, it is basically about Stories that have high level of uncertain, like having 100 Story points. 🤦 When you are managing risks you basically use two tools Definitions of Ready (DoR) - which defines that you tasks are Ready to be taken to sprint (eg. It is required to have less than 10 story points in task estimate) or Definitions of Done (DoD) - which defines that all needed work on your Story is complete (eg.
